25 | On October 2, 2023 at 2:00PM, Licensing Program Manager (LPM), Garfield Leung and Licensing Program Analyst (LPA), April Cowan, met with licensee, Zoe Paul and new site director Rochelle Domingo for an informal office meeting.
The purpose of the meeting was to discuss the self reported unusual incident that occurred June 28, 2023.
A child was found to be missing during lunch time at the building courtyard. Staff and previous director were present at the time. Director states that she believes the child exited the group during a transition from the courtyard bench to children sitting at the lunch table. Per director, child exited the courtyard, through the Brannon St. door, and child headed toward the school when child was found by a security guard from the neighboring Air BnB company located across the street. Per previous director, one guard waited with child at the nearby pizza cafe while another guard went to the facility to see if the child belonged in care there. The child was retrieved by staff. A case management by LPA was conducted August 18, 2023.
LPM and LPA reviewed previous incidents where a child was left at the park. Based on interview with licensee, facility did not make sure that all children were accounted for before leaving the park resulting in absence of supervision.
LPM and LPA reviewed previous incidents where of lack of supervision where a child sustained an injury on their head.
LPM and LPA reviewed proper ratios when away from the facility.
LPM Leung discussed the citation and civil penalty issued for incident. Plan of correction are as follows:
- Previous site director agreed to train staff regarding close supervision of children.
- Licensee held a mandatory staff meeting for all preschool staff on 6/29/2023, 8/12/2023 on-line and 8/25/2023, reinforcing the importance of supervision and head count protocols.
- Licensee met with all staff to review new supervision policies. Licensee has institute an automatic dismissal for lack of supervision.
- New policies have been sent to parents and have signed new policies.
- Children no longer eat in the courtyard, but children will eat inside facility.
- Head counts are done every 10 minutes.
Ms. Paul and Director Domingo were advised that the facility will be visited more frequently.
Exit interview conducted and report was reviewed with the licensee, Zoe Paul and site director, Rochelle Domingo.
This report is emailed to licensee and site director with a request for reply demonstrating receipt.
