Twenty two children, S1, S2, and three staff members (S3, S4, and S5) were present at today's visit.

LPA reviewed the sign-in/sign-out sheet, where parents signed their children into the facility, and found that all signatures were accounted for.

LPA verified with the CDSS Background Check System that S1, S2, and the three staff members have current criminal record clearances and are in eligible status. For today's visit, the facility is observed within capacity limits and ratios.

The facility license, waiver, Child Passenger Restraint System Poster, Notifications of Parents’ Rights, Emergency Disaster Plan, and Personal Rights were displayed inside protective sleeves next to the door.

The last emergency disaster drill was conducted on May 20, 2025, and documented in the facility log. A review of the facility’s previous drill confirmed the facility did not conduct an emergency disaster drill within six months of the most current drill. The last drill was conducted on August 20, 2024. Therefore, the facility will receive a technical violation.

The facility has a first aid kit, a centralized smoke and carbon monoxide detector, and two fire extinguishers.

• Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.

• Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

The facility was equipped with age-appropriate toys, furniture, and learning materials. Each child had a designated storage area labeled with their name. Child safety covers were fitted on electrical outlets. A diaper-changing table was positioned within arm’s reach of a sink. The bathroom was clean, fully operational, and with essential sanitation supplies.

The facility has an outdoor waiver approved by the Department, which allows no more than nine children at a time. LPA observed a group of eight children with one staff member in the outdoor playground, in compliance with the outdoor waiver. The outdoor playground was clean and free of debris, with a fence at least four feet tall. Play equipment was in good condition, with no sharp, loose, or pointed parts. No pools, spas, or bodies of water were present on the property.

The facility provides morning and afternoon snacks for its children. Parents are required to provide their child's lunch. The facility provides filtered water for its children.

The facility provides mats for children to take naps in. LPA observed that sheets and blankets were available for the children. The facility provides sheets to its children, washing them weekly. The facility requires parents to supply blankets for their children, which must be laundered weekly.

LPA reviewed ten children’s files and found that each record is complete. The files contained emergency identification information and required immunizations.

LPA reviewed three staff files and found that all three have current Pediatric First Aid/CPR certificates.

LPA reviewed three Mandated Reporter Training certificates and found that two (S4 and S5) did not have current certificates. Therefore, the facility will receive a technical violation.

LPA reviewed three staff immunization records (MMR and Tdap) and found that one (S5) did not have a record of her required immunizations in her file. Therefore, the facility will receive a technical violation.

LPA reviewed three health screening reports and found that one (S4) did not have a record of it in her file. Therefore, the facility will receive a technical violation.

LPA reviewed transcripts for three staff members and confirmed that two have completed twelve Early Childhood Education (ECE) units, qualifying them to work as teachers. The remaining staff member serves as an assistant.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

Facility Representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. Facility Representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
