25 | On June 3, 2026, Licensing Program Analyst (LPA) Van met with Director Nicky Chin to conduct a case management inspection regarding a reported breath-holding spell involving a child. The purpose of the inspection was explained, and entry to the center was granted. The center is currently operating its Summer Program. At the time of the inspection, the director and ten teachers were supervising 25 children. The center was in compliance with its licensed capacity, limitations, and teacher-to-child ratios.
The center self-reported that an incident occurred when the child became upset after being prompted to complete a routine task. Staff immediately provided physical support, maintained close supervision, and monitored the child’s breathing and responsiveness. The child briefly lost consciousness and then regained awareness without further incident. As a preventive measure, emergency 911 was called.
The director reported that staff were aware of the child’s history of breath-holding spells, based on a similar incident six months ago and a doctor’s note that the team had previously reviewed. Staff had been instructed on appropriate responses, including recognizing early signs of distress, giving the child space when upset, and using calming measures such as a cold towel or ice pack. However, in both incidents, the episode occurred suddenly, and the child held their breath and lost consciousness within seconds.
The director reported that staff follow the doctor’s recommendations in the classroom by attempting to avoid known triggers, offering distraction techniques, and using soothing methods to support regulation. Staff also implement preventive strategies by allowing the child time and space to calm and by monitoring signs of emotional escalation.
Based on the information obtained during today’s inspection, no violations were observed.
A Notice of Site Visit was issued and must remain posted for 30 days.
An exit interview was conducted, and the report was reviewed with Director Nicky Chin.
