25 | During an unannounced Case Management inspection on October 23, 2023, Licensing Program Analysts (LPAs) Van and Tso met with the Site Director, Gilda Villaflores. The inspection followed up on an unusual incident and discussed preventive measures to ensure the safety and well-being of all children attending the facility. The facility has seven staff members and a Site Supervisor who supervises 73 children today.
On October 13, 2023, an incident was reported at the facility where three children, C1, C2, and C3, had left their classroom and headed towards the main gate. Upon investigation, it was found that the teacher in the classroom was aware of the children leaving and had followed them immediately, calling them back. The office assistant was also aware of the incident and had asked for assistance from all staff. All three children were returned to the classroom within a few seconds. After reviewing the Center’s video footage and considering this new information, no deficiency was observed.
Furthermore, the Center has installed a child-proof gate at the classroom entrance. However, the Director will contact the Fire marshal to ensure the child-proof gate complies with the fire code. In addition, the Center plans to have refreshment training for all staff on active supervision to prevent such incidents from happening again.
Today's report, October 23, 2023, and site notice were provided to the Director. LPA informed the Director that the site visit notice and deficiency page must be posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
