On May 21, 2024., Licensing Program Analyst (LPA) Van met with Stefanie Young, who granted LPA entry to the Center. During the visit, the Director, Gilda Villaflores, arrived. The purpose of the inspection is to follow up on an unusual incident and discuss preventative measures to ensure the safety and well-being of all children at the Center. Eleven teachers and a music teacher supervised 83 children.

The facility self-reported an incident on May 8, 2024, in which a child (C1) was left in the bathroom alone without supervision, which occurred at school on May 7, 2024. According to the report, the incident happened when a group of children were transitioning from the bathroom to the classroom. Unfortunately, the staff failed to adhere to the transitioning protocol, leaving C1 unaccounted for and alone in the bathroom.

Since the incident, the staff person involved has resigned. The Center has also taken proactive steps to address the issue, including a thorough review and training session on the importance of supervision and transitioning protocols.

Based on the interview and the incident report received on May 8, 2023, regarding the above incident, supervision was absent, resulting in a child being left unattended in the bathroom alone. As a result, the following Title 22 deficiency was cited on the next page.

A notice of site visit was given and must remain posted for 30 days.

An exit interview was conducted, and the report and appeal rights were reviewed with the Director, Gilda Villaflores.

The Director and Center took Immediate action to correct the incident. The Director reviewed and retrained all staff on supervision, accountability, name-to-face, and passport procedures. Training materials were provided to LPA during the visit. In addition, the staff person involved in the incident has since resigned. The deficiency is cleared.

Based on the interview and incident report, the staff failed to provide adequate supervision and adhere to the transitioning protocol, leaving C1 unaccounted for and alone in the bathroom. This poses a potential health and safety risk to children in care.
