On March 17, 2025, at 2:00 PM, Licensing Program Manager (LPM)Leung and Licensing Program Analyst (LPA) Van conducted an informal meeting with the Center Director, Laysan Andrews, Center Director of Compliance, Ana Field, and Regional Director, Lauren Martin. The purpose of this meeting was to review the center's recent unusual incident report, evaluate the medical emergency protocols & procedures and active supervision, and discuss the requirements for electronic record keeping and reporting requirements.

The following was reviewed, discussed, and reminded:
· The center self-reported an unusual incident that occurred at the center on February 10, 2025, where a child was found nonresponsive when the staff called their name. The child had vomited and urinated on themselves. According to the center representatives, there were no signs or sounds made by the child.
· Following proper safety and emergency response measures, use good judgment
· Notify the parent about the circumstances and the measures implemented. However, emergency services must be called, regardless of the recommendations from the parents or family.
· Supervision regulations: Children must be supervised at all times. No child, while present in the facility, shall be left unattended by a staff member.
· During nap time, perhaps lower the white noise, lullabies, or calming songs so that staff can hear children when they need assistance.
· Electronic Sign-in and Out: In addition to a unique code provided, the person signing must use a legal signature, or the app must capture the full name along with a timestamp of the individual signing the child in and out.

· Reporting requirement: A report must be submitted to the Department by telephone or fax by the next working day during normal business hours. Additionally, a written report that includes the information specified in (d)(2) below must be submitted to the Department within seven days of the occurrence of such an event.

During the informal meeting, type A and B deficiencies were cited. The type B was for failing to call 911 promptly when a child was experiencing a medical emergency. The type A was for lack of supervision, as the child was found vomited, urinated, and nonresponsive during nap time; staff only realized this when the light was turned on.

Since the incident, the Director has verbally reminded all staff of the center’s emergency procedures. Additionally, On March 3, 2025, an official staff training on safety and emergency procedures. All staff in attendance signed and dated.

Since the center has taken action and trained all staff on its emergency procedure, the type B deficiency has been cleared.

The plan for addressing the type A involves the center providing supervision training for all staff. Training materials, staff members’ signatures, and dates to acknowledge their attendance will be sent to the Department to resolve the deficiency.

Facility representatives acknowledge that the Department will increase center monitoring over the next 12 months. Assembly Bill 633 (Childcare Parent Notification Requirements) and Acknowledgement of Receipt of Licensing Reports (LIC9224) were addressed.

LPA Van informed center Director Laysan Andrews, Director of Compliance Ana Field, and Regional Director, Lauren Martin, that this report, dated March 17, 2025, document(s) one Type A citation(s), which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA Van informed the Director and Director of compliance to provide a copy of this licensing report dated March 17, 2025, that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of the Licensing Report (LIC 9224) or other written statement must be placed in the child's file for verification.

An exit interview was conducted, and the report was reviewed with the center Director, Laysan Andrews, Director of Compliance, Ana Field, and Regional Director, Lauren Martin.

The center took immediate action and verbally reminded all staff of the center’s emergency procedures. Additionally, on March 3, 2025, an official staff training on safety and emergency procedures took place. All staff attending the training signed and dated, acknowledging that they had received the training. This deficiency is cleared today.

Based on the interview and record reviewed, the center did not comply with the section cited above. The center did not call 911 promptly when a child was experiencing a medical emergency on February 10, 2025. Failing to take proper safety and emergency response measures poses an immediate health and safety risk to children in care.
