25 | On October 27, 2023, Licensing Program Analyst (LPA) Van met with the Licensee, Lyuba Shkolnik, for an unannounced inspection of the Plan of Correction (POC). Co-Licensee Yanina Nemirovskaya was not present during the visit. The purpose of the inspection was explained, and the Licensee granted LPA entry to the home. There were ten preschool-aged children in care with the Licensee and a helper today.
During the October 4, 2023, annual inspection, the Licensee was cited for several deficiencies. One of these was that both Licensees did not maintain immunization records for themselves and their helper. Additionally, a Type A citation was also issued, as the Licensee’s helper provided care for ten preschool-aged children while both helpers were not at home during the annual visit.
In today's inspection, the records for all staff were reviewed. The records confirmed that all staff members have their immunization records completed and on file. Furthermore, the licensee adhered to her licensed capacity and limitations today. During today's inspection, LPA only observed ten preschoolers supervised by the licensee and her helper. All the deficiencies cited on October 4, 2023, were cleared today.
An exit interview was conducted with the Licensee, and a consultation was provided. In today's inspection, no deficiencies were observed. The Licensee, Lyuba Shkolnik, read and signed the report. Today's report and notice of the site were provided to the Licensee. LPA informed the Licensee that a site visit notification must be posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
