On January 18, 2025 at approximately 9:30AM Licensing Program Analyst (LPA) Katie Krenn arrived for an unannounced annual inspection. LPA met with the Site Director, Ms. Noelle McGregor (D1) and explained the purpose of today's visit. Present at the facility in addition to the Site Director, were five staff supervising 26 preschool children (ages three to entry into the first grade). All staff present and working directly with the preschool children at the facility during the visit have been fingerprinted and background checked by the Department of Social Services and are associated with the facility.

Days and hours of operation are Monday through Friday from 8:00AM - 5:00PM.
This is a nut free facility.
Per D1, there are no pools, spas, or bodies of water on the property.

LPA observed that all required documents, such as the facility license, notification of parents’ rights, car seat laws, earthquake preparedness checklist, and monthly menus, were displayed and visible in the classroom where children are dropped off and picked up by parents and caregivers.

LPA observed that each classroom has a smoke alarm which are part of an integrated system throughout the campus, which is inspected by the fire department. LPA observed several carbon monoxide detectors throughout the classrooms. LPA tested them and found them to be functioning properly. LPA observed that there was at least one fully charged fire extinguisher and first aid kit in each classroom. The most recent emergency drill was January 2026 and LPA observed that it was documented.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

The LPA inspected the indoor and outdoor areas for health and safety hazards. The facility was clean, safe, and well-maintained. Disinfectants, cleaning solutions, and poisons were securely stored out of children's reach, and all accessible cabinets and drawers in the classrooms were free of hazardous materials.

Each classroom has an emergency preparedness backpack with necessary supplies, including a first aid kit, and medical and contact information for each child. LPA observed the teachers carry the backpack with the class to the playground as well as on nature walks to the local park, so they can be prepared in the event of emergency. Children carry their water bottles with them.

LPA observed that the classrooms had age-appropriate toys, furniture, educational materials, and designated storage space for each child, which included cubbies and hooks which were labeled for each child. LPA observed that electrical outlets were covered to prevent children’s access. The upstairs classroom has a bathroom with a sink and toilet. The bathroom downstairs classroom has four child sized toilets and sinks. LPA observed that the bathrooms were clean, fully operational, and equipped with suitable toileting equipment and sanitation products. LPA observed separate bathrooms available for staff.

There is an outdoor play area with a sandbox, which is covered when not in use. LPA observed that the sandbox was clean. Per D1, they do not have a problem with keeping it clean. LPA observed that the outdoor play equipment is suitable for children ages three to entry into the first grade.

The outdoor play equipment is in good condition and does not have sharp, loose, or pointed parts. There are a variety of bikes, blocks, and other outdoor toys available. The areas around and under climbing equipment and slides are cushioned with artificial grass to prevent injuries from falls. LPA did not observe any pools, spas, or bodies of water on the property. The facility does use a water table on hot days and per D1, it is drained after each use.

The facility has a catering service that provides lunch and snacks. Some parents pack lunches and snack from home for their children. This facility is nut-free. Per the D1, currently none of the enrolled children have any allergies. At this time, the facility does not administer any medication to the children.

The facility offers mats for children under the age of five to rest or nap. Each child has their own. LPA observed that bedding from home is available for children. Per D1, the bedding goes home weekly for laundering and the facility cleans the mats.

LPA reviewed the files of five children, confirming that their emergency contact and medical information were complete and in order.

LPA reviewed five staff files and confirmed that all necessary forms were complete and in order. LPA verified that multiple staff onsite held current Pediatric First Aid and CPR certification. LPA found that all teachers held current mandated reporter training certifications.

Facility Representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Facility Representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
