25 | On January 23, 2026, Licensing Program Analyst (LPA) Leong arrived at the facility to conduct an unannounced case management visit. This visit was to follow up on two self-reported incidents by the facility. LPA met with Director Nancy Lambert-Campbell and explained the purpose of the visit.
During today’s visit, there were 10 staff members and 45 children present.
On December 1, 2025, an incident occurred in which a child was left unsupervised in a classroom. According to the facility, the child was hiding under a table while staff were escorting other children to lunch. Based on the discussion with the director, the child remained under the table during the transition to the cafeteria and was not immediately noticed by staff.
On December 17, 2025, a second incident involved three children. Two children allegedly punched another child; however, after reviewing the incident with the director, LPA determined that the children were playing and that staff intervened immediately to address the situation. No further action is required for this incident. LPA reminded the director that supervision regulations must be consistently met to prevent such incidents.
Based on the information available, the facility will receive a deficiency related to the December 1, 2025 incident.

A notice of site visit was given and must remain posted for 30 days.
