Licensing Program Analyst (LPA) Haydee Caliboso met with applicant Laura Wong (Owner/Director) today for a pre-licensing inspection. The applicant has requested a combination of an infant with toddler component program. The applicant has requested 20 infants, ages birth to 18 months, and 10 toddlers, ages 18 months to 36 months. The facility plans to operate Monday - Friday; 7:00AM to 6:00PM. The designated Site Director for the facility will be Laura Wong. The facility was inspected today, indoor and outdoor, for health and safety hazards, and was measured to calculate capacity. This inspection included a technical assistance inspection for COVID-19 guidance and support.

Indoor: The facility building that will be used for the program is on the lower level of a residential building. The facility building shares the entrance with the tenants who are living on the 2nd floor of the building. There is one designated classroom for the infant program and one classroom for the toddler program. The infant classroom was calculated to be a total of 733.94 sq. ft., allowing for a capacity of 20 infants. Toddler classroom was calculated to be 417.82 sq. ft., allowing for a capacity of 10 toddlers. The classrooms are observed today to be equipped with a fire extinguisher, and both smoke and carbon monoxide detectors. The facility is adequately ventilated and free of insects and bugs. The classrooms and furniture equipment are clean and in good condition. Children’s toys and supplies are orderly, in good condition, and age appropriate. There is storage space for children's personal belongings located at the Staff lounge area. LPA reminded the Director that no children are allowed in the Staff lounge area to access the storage space during the hours of care. Director stated that only parents can access the storage space during drop off/pick up. Staff will bring children’s personal belongings into the classroom by using a pushcart. Each classroom has sinks and toilets available for children to use. The facility has a first aid kit in each classroom and a designated backpack with emergency supplies. LPA reminded the Director that infant children and toddler children cannot co-mingle.

Refrigerated medications will be stored separately from food. Medication will be administered with parental consent and will be kept in the child’s file for record. All medication will be logged when administered to children. The facility has a designated area with a working refrigerator. The program will provide morning and afternoon snacks daily. Parents will provide lunch daily. The program will provide a sheet for the children. Bedding is to be washed twice per week or as needed. Cots will be used for older children and will be cleaned daily or as needed. The toddler classroom has 1 toilet and 1 sink available for children to use. The bathroom was observed with no health or safety hazards.

Infant: The program will provide diapers and wipes for the infant. The staff will ensure breastmilk and or milk formula is served fresh daily to the infant. The infant classroom has a designated refrigerator inside a closet located in the infant classroom, which includes bottle warmers. The bottle warmer is out of reach of the children. There are 6 cribs available for infant to use. Director stated additional cribs will be purchased as enrollment increases. Drinking water for the children will be provided by plastic baby water bottles or cups with lids provided by the program. LPA reminded the Director that commercially prepared baby food in jars shall be transferred to a dish before being fed to the infant. Bottles, dishes and containers of food brought by the parents shall be labeled with the child’s name and current date. Medications for infants will be stored in the classroom. All furnishings and equipment are observed to be safe, in good condition, and age appropriate. The diaper changing table area has a sink within arm’s reach. The infant classroom has 2 sinks and 1 toilet. Staff will meet and review the individual written needs and service plans quarterly with the parents. Plans will be kept in the children’s files.

There is one restroom available for staff to use. The isolation area for ill children is located at the staff lounge area. Staff lounge and staff bathroom will be used only for ill children, if needed, until parents arrive for pick-up. LPA reminded that supervision is required while children are waiting for parents to arrive for pick-up. Children in the program will be signed in and out daily by parents using an electronic signature. The facility will conduct and log fire drills once every six months, and the log will be available upon request. All cleaning supplies and hazardous items are stored in a locked cabinet made inaccessible to children.

Outdoor: The facility has no outdoor play area. The program has submitted an open space playground waiver to San Francisco Park and Recreation Department to use the children’s playground at Argonne Playground located at 463 18th Ave, San Francisco which is 0.2 miles away from the facility.

The program has obtained an approval of a permit from SF Park and Recreation Department for playground usage until 12/30/22. The program will ensure permit for park usage is current and up to date. The park was inspected and meets the requirements for the open space waiver. Children will be transported to the park using buggy baby stroller. LPA reminded the Director; sandbox will be inspected daily and kept free of hazardous materials. LPA reminded the Director to ensure ratio and supervision are observed at all times. The program will provide a portable tent for children to use for shade structure while at the park. The Director was reminded and instructed to use only one side of the playground structure that is age-appropriate for children in the program. Please see photo of the playground structure kept in file. The program will provide field trip permission signed by all parents as part of enrollment agreement. Director will provide policy and procedure to ensure staff are stationed while at the park for best practices of supervision. Additionally, the program will provide water for outdoor time.

Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with applicants and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed [applicant, licensee, or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Exit interview conducted and report was reviewed with the applicant Laura Wong.

The following corrections need to be made and submitted for further review before a recommendation for licensure with a capacity of 33 can be made:

· Obtain approval from SF Fire Department.
· Letter for scheduled playground/outdoor rotation for toddler and infant program.
· Trash cans with tight fitting lids.
· Photo of posting requirements are posted.
· Ample age-appropriate toys for toddler and infant children.
· Policy and procedure for staff for park usage.
· Policy and procedure for parents to access children’s belongings located in the staff lounge area.
· All clutter in the infant closet where milk/food will be stored must be cleaned, organized, and free of dust. All extra supplies must be stored properly.
· All items for storage located in the staff lounge room needs to be cleaned and stored properly.
· Shelves, infant and toddler activity space area, common entrance hallway floor, and restrooms must be clean and free of dust.
· Provide a written permission as part of Admission Agreement for child’s authorized representative for the placement of their child in the toddler program.
