On March 4, 2026 at approximately 9:00am, Licensing Program Analyst (LPA) Quimbo conducted an unannounced, annual inspection. Upon LPA's arrival, LPA met with teacher, Ana Escobar, and explained the purpose of the visit. Director was not present during LPA's visit due to sickness. LPA observed teacher to be designated with facility responsibility. LPA conducted annual inspection with teacher designated with facility responsibility.

Facility is a combination program with an infant component and preschool component. Hours of operation are Monday through Friday 7:30am to 6:00pm. The infant component operates in two classrooms: "Forest" (3 months to 24 months) and "Mountain" (12 months to 24 months) classrooms.

Under the infant component, present during LPA's visit included 16 infant children and 6 teaching staff: 8 infants in the Forest classroom with 3 teaching staff and 8 infants in the Mountain classroom with 3 teaching staff. Infant program is operating within capacity limits and ratio during LPA's visit.

LPA inspected entire facility for health and safety hazards. LPA observed licensing documents to be properly posted and made available for review in the main entrance of the facility. LPA observed licensing documents includes the facility's outdoor rotation waivers, daily schedules and menus. Required licensing documents were observed to be posted, available for review.

LPA observed infant classrooms to have a variety of toys, materials and equipment that are age appropriate. Per teacher, toys and materials are sanitized daily. Infant classrooms includes areas for auditory, visual, and manipulative development. Furniture for children was observed to be age appropriate and in good condition. There are low, accessible sinks in the infant classrooms that do not provide hot water. Per teacher, sinks are able to provide hot water, however, hot water has been turned off. LPA also observed low cabinets in classrooms to have child safety locks installed.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

There are rugs in the classrooms that are free of stains. Per teacher, rugs and flooring are cleaned daily by facility's janitorial services. Storage for children's belongings are through cubbies that are stored either in the classroom or the hallway. LPA observed children's cubbies to be labeled with their individual names.

LPA observed electrical outlets in classrooms to be made inaccessible with child safety covers. LPA also observed chemicals and cleaning solutions to be made inaccessible, behind child safety locked cabinets or in classrooms' high shelves. LPA did not observe garbage bins in infant classrooms. Garbage bins were observed to be stored in the infants' restroom area.

There is a shared teachers' space in between the infants' classrooms that is off limits to children. There is also a shared children's restroom in between the infant classrooms. LPA observed toileting equipment for potty training infants in restroom to be age appropriate. Restroom was observed to be in working condition. There are diaper changing tables in the restroom that is within arm's reach to an adult sink. LPA observed diapers and wipes to be labeled with children's individual names. There is a garbage bin in the restroom with a tight fitting lid to dispose of dirty diapers.

Facility includes multiple fully charged fire extinguishers, fire alarm, fire sprinklers and smoke detectors. Per teacher, detectors are combination smoke and carbon monoxide detectors. LPA was unable to test detectors due to placement in high ceiling. LPA also observed classrooms to also be equipped with first aid kits, an emergency backpack, and emergency binders with children's emergency contact information. There are also emergency supplies in outdoor area of facility.

Food service is provided to the older infant children through a third party vendor that is delivered daily. Children's families provide younger infants with their food service that includes specific milk and formula. Per teacher, food is prepared in the shared teachers' space. LPA observed shared teachers' space to be equipped with a dishwasher, microwave, and refrigerator. LPA observed young infants' personal milk, formula, and bottles to be individually labeled and stored appropriately in the food preparation area and refrigerator. Reusable utensils and plates are utilized for older infants and washed daily in facility's dishwasher. LPA did not observe utensils or food to be stored next to cleaning solutions.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP.).

LPA verified that the lead testing was completed in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP. The Forest classroom has a designated napping area for infants who sleep in cribs. LPA observed napping area to be physically separate from the infants' activity area. Cribs do not hinder the entrance or exit to the classroom. Cribs are also labeled with a specific infant's name. LPA observed an infant napping in a crib during visit. LPA observed infant to be sleeping in a crib free of loose articles and objects.

Infants in the Mountain classroom sleep on cots. LPA observed cots to be appropriately stored in a closet, when not in use. LPA also observed children's cots to be labeled with their individual names. Children bring their own sheets and blankets. Sleeping logs are maintained for infants. LPA observed sleeping logs document the initial time the infant falls asleep, the 15 minute time check, the time the infant wakes up and any signs of distress. LPA reviewed sleeping logs from random months of March 2025, July 2025 and October 2025.

LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA observed children's sheets to be stored in their cubbies when not in use. Per teacher, sheets and blankets are sent home weekly or as needed for laundering. There is a washing machine and dryer on site, if needed.

Infant program has an outdoor area that is separate from the preschool component and fully enclosed. Outdoor area includes a variety of outdoor toys and materials that are in good condition. There is soft padding for young infants. Outdoor rotation waiver is posted in facility. There is a sandbox in the outdoor area that was observed to be clean. Per teacher, sand is replaced every so often to ensure cleanliness. Drinking water is made available to children through their individual water bottles that can be refilled on site through filtered sinks. LPA observed children's water bottles to be labeled with their individual names.

Infant program utilizes an electronic sign in and out application. Per teacher, each family has a unique code that is specific to the family. When an individual utilizes code to sign a child in or out, the individual signs their legal signature on the electronic application. LPA observed children to be appropriately signed in during today's visit. LPA also reviewed a random sign in/out sheet from a random day in February 2026. Sign in and out sheets are maintained and made available for review. Teaching staff also maintain a manual sheet of children who are present, daily.

At approximately 11:30am, executive director, Anne Senores, arrived to the facility. Program conducts emergency disaster drills at least once every six months which are documented. Last documented disaster drill was conducted October 15, 2025. A childcare roster is maintained in each classroom. A roster was also made available for LPA's review.

LPA reviewed 6 random, infants' records, which were complete with physicians' reports, immunization records, completed admission agreements, infants needs and services plans and Individual Infant Sleeping Plan forms (LIC9227s). LPA also reviewed infant teaching staffs' records, which were also complete. Teaching staffs' records include required immunization records, educational transcripts and/or teaching permits, current Mandated Reporter certificates and current Pediatric CPR and First Aid certification. Multiple staff present today's date have a current CPR and First Aid certification. LPA also confirmed infant teaching staff present have units related to infant and toddler care to ensure teacher to child ratio was met during today's visit.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
