13 | On July 23, 2025 at approximately 1:00PM, Licensing Program Analysts (LPA) Melissa Zaragoza conducted an unannounced complaint visit, to deliver findings on complaint investigation, for the above allegations. LPA met with director, Sophia Adler, and the purpose of the visit was explained. Present during LPA’s visit included 4 staff, and 13 enrolled preschool children. All adults working in the facility with children have fingerprint clearance on file.
During the investigation process, LPA conducted multiple walk-through inspections, collected pertinent information, reviewed facility staff records, reviewed facility children’s records, conducted interviews, and conducted an annual inspection visit. After the investigation was conducted, LPA discussed with the director the food service options for children. LPA reminded the director to have children’s families complete the children’s Meals Accomidations form.
After investigating the second allegation, 'staff allow a daycare child to be soiled while in care’, LPA reminded director to complete the diaper log form, for children enrolled that still need diapering. Based on walk through inspection, children’s bathrooms were observed to be in the classroom, and children were free to use it when they had to, with a staff member.
Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is unsubstantiated.
Exit interview was conducted and report was reviewed with Director, Sophia Adler. A copy of this report was provided. No deficiencies were made today under Title 22 Division 12 of the California Code of Regulations.
NOTICE OF SITE VISIT WAS GIVEN AND SHALL REMAIN POSTED FOR 30 DAYS. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
