25 | On November 15, 2024, LPA Nathan Garcia conducted an unannounced, plan of correction visit (POC) to the facility. LPA met with licensee, Munkhzaya Sengee, and explained the purpose of the visit. Present during LPA’s visit included 9 children in care including two of the children are the licensee's. The licensee's adult daughter and husband were also present.
On October 30, 2024, LPA conducted an unannounced, annual visit at facility. During the visit, LPA observed that there were 6 infant children and 5 children at the age of two, with no helper until after 2:00pm when her teenage daughters arrives. Facility was issued a Type A citation and was cleared during the visit due to her daughters arrival. A plan of correction and signed LIC9224s were discussed with Licensee.
Plan of correction was for licensee to ensure that ratio is met at all times and capacity regulations are met.
As of this date, LPA observed completed LIC9224s for 8 enrolled children, additionally 2 of her children are also part of the ratio. LPA observed notice of site visit given during annual visit to be posted and available for review. LPA also reviewed that LIC 282, 9224, Sleep logs and children's record are on file.
As of today's POC visit, the licensee is in compliance. There were 3 infant children and the rest of the children are over the age of 2. One child turned 2 years old on 11/3/24, one child is part time and not present, while the other has been disenrolled from the facility. The proof of removing the child from the program were text messages between licensee and mother were sent to LPA on 10/30/2024.
Notice of site visit was given and must remain posted for 30 days.
