25 | Licensing Program Analyst, LPA Yee conducted a pre-licensing inspection today. LPA met with the licensee, Yolanda Amador and her son, Juan. This is a change of location from 1050 Capitol Ave, SF, CA; license number 384004421. LPA inspected the entire home with Yolanda. Current residents are Yolanda, her two grandchildren ages 14 years 9 years old daughters. Days and hours of operation: Monday to Friday between 7:30 AM to 5:30 PM.
Day Care Areas (lower level): living room, bedroom #1, bedroom #2, bathroom #1, bathroom #2 and backyard. The off-limit area: The entire upper level, garage. Licensee leased this home. The lease agreement is obtained during the inspection. The home is equipped with a carbon monoxide detector, smoke detector, and fire extinguisher that meets the fire department standard are available. LPA advised the to conduct emergency drills once every six months and keep a log sheet.
The licensee has required immunization records on file. CPR and 1st aid are current until 7/17/2023.

LPA review AB 1207 with the Licensee. As of January 1, 2018, all staff members are required to complete Mandated Reporter Training every two years. The training can be obtained online at www.mandatedreporterca.com. AB 1207 certificate for the licensee is on file . The facility has a small dog and kept in the off limit areas (upper level).
