Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. Exit interview conducted and report was reviewed with Helena Geng & Padma Alamuru.
An electronic copy of this 4 page report was emailed to Helena Geng. Applicant was asked to review the report and return a signed copy of the report to LPA Haydee Caliboso.
The following corrections need to be made and submitted for further review before a recommendation for licensure with a capacity of 18 children can be made:
· Obtain approval from SF Fire Department.
· Trash cans with tight fitting lids.
· Photo of posting requirements are posted.
· Policy and procedure to ensure children are supervised when transporting infant children to and from the park.
· Policy and procedure to ensure infant children are supervised while at the park.
· All clutter in the infant classroom must be cleaned, organized, and free of dust.
· All extra supplies must be stored properly.
· Photos of sleeping area physically separated from indoor activity space. |
