Applicant is aware of teacher to child ratio when engaging in activities away from center, including utilizing local park, is 1 adult to 6 preschool children.
LPA observed both playgrounds to include play structures that have resilient padding underneath. LPA informed applicant, facility will only be licensed for basketball court and playground #2. Playground #2 was observed to be fully fenced and age appropriate, equipped with a a play structure and sandbox. Applicant is aware sandbox must be clean prior to enrolled children's use. Applicant is aware playground #1 is not to be utilized for preschool children due to play structure not being age appropriate.
LPA reminded emergency disaster drills are to be conducted at least once every six months. LPA reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.
Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

-Fire clearance.
-Inaccessibility of bricks in classroom.
-Child safety lock on staff room.
-Installation of sound alarm on exit doors.
-Management approval.
