25 | On May 14, 2024, at approximately 11:15am, Licensing Program Analyst (LPA) Ly conducted an Unannounced Case Management-Licensee Initiated Inspection for an increase capacity from a small family child care to a large family child care. LPA met with Licensee Bohdan Stryuk. Purpose of visit was explain. Present during the visit were Licensee and an Assistant caring for 2 preschool age children.
During today's visit, LPA and Licensee inspected the entire home. On this day, LPA inspected the back yard and found a wooden stair has been added in the yard. Per Licensee, stair was added to meet Fire Code. LPA also observed there's an on going construction to turn the garage into another living space. Per Licensee, garage will be converted into living space for family member and garage's living space has been granted permit by the Department of Building Inspection and will bear address 235A Lincoln Way.
Based on observation and interview, Licensee did not inform the department in regards to adding exterior stair in the backyard and the conversion of the garage in the home poses a potential health and safety risks to children in care. Licensee was advised to notify the department for any alteration of the building/home before construction begins. Type B deficiency being cited based on LPA Ly’s observation in accordance with the California Code of Regulations, Title 22, see LIC 809D.
Fire Clearance for the Large Family Child Care has been granted by the San Francisco Fire Department on 04/16/2024.
Per Licensee, day care areas are Living room as Playroom, dining room, Kitchen, bedroom for Nap room, Bathroom #1 and Bathroom #2 and the Yard. Off Limit Areas are Licensee's Bedroom, the laundry room on the second floor. Laundry room is to use as a passage way for children to go down to the yard. Off limit on the ground floor are the entire ground floor and the garage.
