On February 27, 2024 at approximately 10:30am, Licensing Program Analysts (LPAs) Catrina Quimbo and Jamel Maiwandi, conducted a scheduled pre-licensing inspection. LPA met with applicant, Queenie Lee, and explained the purpose of the inspection. Present during visit included 6 staff and 17 children.

This is a change of ownership application. Facility is currently licensed and operating under facility 384000353. Application was received January 25, 2024. As of this date, fire clearance has not yet been obtained. Applicant is requesting for a preschool program for a capacity of 26 children (2 years to entry into first grade).

The facility will operate Monday through Friday, 7:45am to 5:30pm. The program will operate in a multi-level building on the ground floor only, in the outdoor area and 4 classrooms. The designated director for the facility will be Queenie Lee. The facility was inspected indoors and outdoors, for health and safety hazards, and measured to calculate capacity.

Indoors: There are 4 classrooms that will be utilized for the preschool program: Classroom #1, Classroom #2, Classroom #3, and Classroom #4. Classroom #1 was measured for 127.37 sq. ft. allowing for a capacity of 3 children. Classroom #2 was measured for 157 sq. ft. allowing for a capacity of 4 children. Classroom #3 was measured for 156.76 sq. ft. allowing for a capacity of 4 children. Classroom #4 was measured for 318.76 sq. ft. allowing for a capacity of 9 children. Total indoor square footage equals 759.89 sq. ft. allowing for a capacity of 21 children.

Facility also includes a lobby area that is utilized for cubby space and a sign in/out area. There is also a kitchen/staff room that is a pass by area for children. LPAs observed classrooms to be ventilated with appropriate temperature. Classrooms are equipped with furniture, toys and materials that were observed to be age appropriate and in good condition. Flooring in classrooms include carpeting and hardwood floors that were in good condition. Garbage bins were observed to have tight fitting lids.

LPA observed electrical outlets in classrooms to be made inaccessible, with child safety covers. Poisons, cleaning supplies, and hazardous materials were observed to be stored in classrooms' high shelves. Facility is equipped with an at least one working carbon monoxide detector. LPA tested detector next to children's restroom during visit, which was observed to be working. Applicant is currently in the process of installing a new fire alarm system. LPA observed facility is equipped with a fully stocked first aid backpack. Per applicant, medications with children's individual names and required documentation will be stored in the lobby.

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. Program plans to provide napping children with mats. Children's families are to provide blankets and sheets. Per applicant, blankets will be sent home weekly or as needed. Applicant stated mats are sanitized daily.

Applicant plans for isolation area to be in Classroom #1, separate from other children. LPA reminded applicant children must always be supervised. Program's discipline policy was discussed. Applicant plans for children to be redirected and communicating their feelings.

Facility includes one children's restroom that includes 2 toilets and 3 sinks. Total allowable capacity for toilets is 30 children. Total allowable capacity for sinks is 45 children. LPA observed sinks and toilets to be in proper working condition. Sinks children have access to do not provide water that is warmer than 120 degrees Fahrenheit. Applicant is aware children must be supervised going to and from bathroom.

Program does not plan to provide a food service. Children's families will bring food that will include a morning snack, lunch and afternoon snack. There is food available at facility if needed. LPAs observed cleaning supplies in kitchen to be stored behind child safety locked cabinets. LPAs did not observe any accessible sharp objects. Children are required to provide their own individual water bottles that will be labeled with their individual names that can be refilled at center.

The requirement for Lead Water Testing was discussed (H&S Code 1597.16). Applicant was reminded a facility licensed after July 1, 2022, must have their water fixtures tested for lead 180 days after licensure and every 5 years thereafter. LPA advised applicant to review PIN 21-21-CCP.

Outdoor: Outdoor area is located behind facility. Outdoor area is entirely fenced and enclosed. Total outdoor area measures for 1,639.63 sq. ft. allowing for a capacity of 21 children. Outdoor area is equipped with a variety of outdoor toys and materials that were observed to be in good working condition. There is a sandbox that includes a cover. There is sufficient shade from neighboring building. There is a separate outdoor area that the program does not have access to and is fenced. Separated outdoor area was not measured during today's visit and is not licensed for program. LPA reminded applicant children must always be supervised going to and from the outdoor area.

LPA reviewed with applicant, the LIC 311A, Records to Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Applicant stated emergency disaster drills will be conducted monthly. LPA reminded applicant to conduct and document emergency disaster drills.

Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Applicant was informed of the MyChildCarePlan.org site, a consumer education
website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep, as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Based on indoor square footage, maximum number program can be licensed for is 21 preschool children.

Prior to recommended licensure, following must be completed:
-Fire clearance.
-Additional resilient padding under play structures in outdoor area.
-LIC184C documents resubmitted.
-LPA's approval of resubmitted LIC184C documents.
-Removal of applicant as a co-licensee for their current licensed childcare home or submission of director packet for facility.

A notice of site visit was given to applicant and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.

Exit interview conducted and report was reviewed with the applicant, Queenie Lee.
