On 11/3/ 2025, Licensing Program Analysts (LPAs) J. Badger and A. Tso, conducted an unannounced visit to deliver the complaint findings at the above-named facility. LPA met with facility director, Danni Feng, and explained the purpose of the visit. Present during today’s visit there were 88 children in care with 12 staff members including the director.

Based on LPA’s gathered information through observation and interviews, the agency has investigated the following complaint allegations: " Staff did not attend to child in care"

Although the allegation may have happened or is valid, there is no preponderance of evidence to prove the alleged violation did or did not occur. Therefore, the allegation is Unsubstantiated.

Appeal Rights were given to the site director, Danni Feng.
Notice of Site Visit was given and shall remain posted for 30 days.
Exit interview conducted and report was reviewed with the director, Danni Feng.

Staff withheld child's pacifier.

The preponderance of evidence standard has been met. The above allegations were found to be SUBSTANTIATED.
Based on the evidence gathered, a Type B violation will be issued today

Based on observations in the field, record review and, the licensee did not comply with the section above. Based on the meeting between the director and staff, the staff member admitted to taking the pacifier from the child at the outdoor play yard, which pose/posed a potential health, safety, or personal rights risk to person in care.
