25 | On July 26, 2024 at approximately 12:00pm, Licensing Program Analyst (LPA) Ly conducted an Unannounced Relocation Inspection to this family child care home and met with Licensee Yevgeniya (Jane) Yurkevich. Purpose of visit was explained. Present during the visit was just Licensee Jane Yurkevich. Per Licensee, there are 6 children in care today and they are at the park with the Assistants. Licensee Jane Yurkevich owns this two story home and resides here alone. There are two bedrooms and 1 bath downstairs; the living room, open kitchen dining area and a full sized bathroom on the second floor. The Two Assistants and 6 children returned to facility 10 minutes into inspection.
Licensee Jane Yurkevich has requested to relocated her license number 384002225 from 1664 11th Avenue, San Francisco to 1662 11th Avenue, San Francisco which formerly was a family child care operated by Licensee Jane Yurkevich's mother, Galina Berenshteyn license number 384002057. Licensee Galina Berenshteyn has surrendered her license to the department for the purpose of retirement.
Day care areas are one bedroom as the entrance to the day care on the ground floor, living room, open kitchen dining room, the bathroom on the second floor and the front yard. Off Limit Area is licensee's bedroom. Licensee’s Pediatric CPR/First is valid until 08/2024. Licensee stated she has scheduled to renew her Pediatric CPR/First Aid. Completed Child Abuse Mandated Reporter Training Certificate is on file and is valid until 08/2024. Required Immunization: Influenzas, Pertussis and Measles are on file. Fire Clearance has been granted by the San Francisco Fire Department under the name Yevegeniya Yurkevich on 07/16/2024.
LPA observed the home is clean orderly and properly ventilated. LPA also observed a 2A10BC Fire extinguisher, operable smoke detectors and carbon monoxide. There are no Fireplace or bodies of water in the home. Electrical outlets have child protective covers in place making them inaccessible to children. Chemical, detergents, cleaning compounds, medications, and other items of this nature are made inaccessible to children. Kitchen/Bathroom cabinets/drawers have child protective locks in place making all sharp objects or toxic house hold items inaccessible to children. First aid supplies are available for children.
