25 | Centralized Application Bureau (CAB) Licensing Program Analyst (LPA) Jamel Maiwandi met with Applicant Siu Chan and Director Sunny Quin for the purpose of an announced pre-licensing inspection at 3101 Clement Street San Francisco, CA, 94121. Applicant requests an infant license to serve 24 infant children from birth to two years of age. The program will operate Monday through Friday from 8:00am to 5:30pm. The fire clearance was granted on 7/19/2024.
Applicant acknowledges that the following documents must be posted at all times: License, Emergency Disaster Plan, Personal Rights, Parents' Rights Poster, Effects of Lead Exposure, car seat poster, menus, and daily schedule. LPA provided and discussed LIC311A and the forms that must be in each child's and each staff member's file. Applicant stated the Needs and Services plans will be kept in the classrooms. Parents will provide formula, breast milk, and baby food for their children.
INDOOR ACTIVITY SPACE:
There is one infant classroom. LPA observed a sufficient number of tables, chairs, and cubbies for 18 infants. LPA observed six cribs and eight napping cots/mats with a partition wall that is at least 4 feet tall. LPA observed a variety of age-appropriate toys and equipment. Applicant acknowledges there will be a maximum of 18 infants under 24 months old, due to available cribs. There is a first aid kit in the main classroom. Medications will be stored in the storage area in a locked box. LPA observed cleaning disinfectants are appropriately stored and inaccessible to children. Applicant stated there are no poisons or firearms on the premises. Applicant stated drinking water will be provided via filtered water in the main classroom and will be provided by parents as well. LPA observed a functional carbon monoxide detector in the main classroom. Applicant stated they will use a paper sign-in/sign-out system.
