Centralized Application Bureau (CAB) Licensing Program Analyst (LPA) Jamel Maiwandi and Licensing Program Manager Mai Lor (LPA) met with Director Raymond Zhong for the purpose of an announced pre-licensing inspection at 3198 Fulton Street San Francisco, CA 94118 on 9/9/2024 at approximately 10:35am. Applicant Michelle Lin was not present during this inspection. Also present was Tony NG and Karen Huang. Applicant requests a preschool component to serve 37 children from age two years to entry into first grade and an infant component to serve 12 children ages 3 months to 24 months, for a total of 49 children. The program will operate Monday through Friday from 8:00 a.m. to 5:30 p.m. The facility is located on the first floor. And above the facility is a multi-unit apartment complex. LPA and LPM observed scaffolding and active construction outside of facility. The director was advised to ensure the health and safety of children while construction and scaffolding is present during the hours of operation.

LPA and LPM reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

INDOOR ACTIVITY SPACE:
There are two preschool classrooms (classrooms A and B), and a food preparation area which will not be used for children’s play activities. There is a storage closet which has a refrigerator. The director was reminded that all kitchen, food preparation and storage areas shall be kept clean and free of litter and rubbish; and measures shall be taken to keep all such areas free of rodents and other vermin. In addition, there are two infant classrooms (classrooms D and F) that will be used by the children located at the facility, and a separate infant napping area (class E). LPA observed 5 infant cribs. Applicant was advised that there shall be max 5 crib infants at any given time. Preschool classrooms A and B contain a total of 985.29 square feet. Infant classrooms D and F contain a total of 333.03 square feet.

LPA and LPM measured the classrooms which includes a total of 1,318.32 square feet and will not accommodate the applicants request for 37 preschool children and 12 infants. LPA and LPM observed a sufficient amount of equipment, toys, tables, chairs, napping mats, and cubbies in each room. There is one first aid kit located in each classroom and one in the main entrance area as well.

Medications will be stored in food preparation area and inaccessible to children. Director stated cleaning and disinfectant agents will be stored in the food preparation area and inaccessible to children in care. Director stated there are no poisons or firearms on the premises. Director stated drinking water will be provided via water bottles and parents will provide drinking water as well. LPA and LPM observed a functional carbon monoxide detector next to food preparation area. Director stated they will be using a digital sign-in and sign out process. Director stated outside of the facility, parents can drive up to the entrance and drop off the children or walk their child in through the entrance. A separate entrance is available for infants located behind infant classroom F. Staff will be present to receive and ensure that parents sign in during the curbside drop off. Director stated parents will provide sheets, blankets, and pillows for their children during naptime. There is a total of two toilets, one urinal and three sinks available for preschool children. There is a separate restroom with one toilet and one sink, and 1 portable foot operated pump sink in infant classroom E, available for infants. Director was reminded to ensure infant sink is in safe, sanitary, and working conditions at all times according to regulation 101239(e)(4). Director stated there will be a dedicated mobile line for the facility. Director stated the nook space will be utilized as an isolation space for sick children. Director was informed facility shall be kept clean, safe, sanitary and in good conditions at all times to ensure the safety and well-being of children, employees and visitors. LPA and LPM observed separate private restrooms for the staff located next to the children’s restroom.

OUTDOOR ACTIVITY SPACE:
There is not an outdoor activity area located in the facility. In lieu of an outdoor activity space, the Director stated the facility will utilize Golden Gate Park to fulfill outdoor play space and have submitted a waiver request to include a safety plan. The park is located across the street from the facility and Director stated they will utilize strollers and safety equipment to ensure safe transportation to and from the park. Director was advised to ensure proper supervision ratios while children are outside.

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. LPA discussed the following: 100% supervision is required at all times, including in the bathroom; personal rights; inspection authority; reporting requirements; staff to children ratios and capacity; staff qualifications; and maintaining buildings and grounds. LPA discussed with director any changes that may occur regarding the director or an employee acting in the director's absence must be reported to department within 10 working days.

Director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Director was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Assembly Bill (AB) 2370, /chapter 676, Statues of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-year after the date of the first test.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP)

Exit interview conducted and report was reviewed with the Director Raymond Zhong.

The following items are required before a license will be issued:

1. Department review of outdoor activity space waiver request to utilize the public park across from the facility in lieu of an outdoor activity space.
