Centralized Application Bureau (CAB) Licensing Program Analyst (LPA) Jamel Maiwandi and Licensing Program Manager Mai Lor (LPA) met with Applicant Ana Field for the purpose of an announced pre-licensing inspection at 301 De Montfort Avenue San Francisco, CA 94112 on 8/20/2024 at approximately 10:30am. Present during this inspection was Site Director Aile Salazar-Ramos, Principal Rachel Altreuter, and Laura Morgan. Applicant requests a preschool component to serve 24 children from age two years to entry into first grade. The program will operate Monday through Friday from 7:00 a.m. to 6:00 p.m. The program will be operating on the Stratford private elementary school ground. The applicant is seeking one preschool classroom to licensed.

LPA and LPM reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

INDOOR ACTIVITY SPACE:
There is one preschool classroom (classroom #106). LPA and LPM measured classroom #106 which includes a total of 695.12 square feet and will not accommodate the applicants request for 24 preschool children. LPA and LPM observed a sufficient amount of equipment, toys, tables, chairs, napping mats, and cubbies in the classroom. There is one first aid kit located in the classroom. There is a functional carbon monoxide detector in the classroom. There are two toilets, two sinks, and one drinking fountain located in the classroom. In addition to the water fountain, the applicant stated water will be provided via water pitcher and cups.

Medications will be stored in the main administration office and medication according to each individual child’s needs will be stored in their individual containers. Applicant stated cleaning and disinfectant agents will be stored in the locked storage cabinets and inaccessible to children in care. Applicant stated there are no poisons or firearms on the premises. Applicant stated drinking water will be provided via a drinking fountain in each classroom and parents will provide drinking water as well. Applicant stated they will be using a digital sign-in and sign out process and a paper sign in and sign out as a backup. Applicant stated parents will provide sheets, blankets, and pillows for their children during naptime. Applicant stated the main administration office will be utilized as an isolation space for sick children. Applicant was informed facility shall be kept clean, safe, sanitary and in good conditions at all times to ensure the safety and well being of children, employees and visitors. LPA and LPM observed separate private restrooms for staff located in the main administration. LPA and LPM observed the food preparation area to be separate from the classroom and located next to the entrance of the outdoor play area and is inaccessible to children via a locked door.

OUTDOOR ACTIVITY SPACE:
There is one outdoor play area on the property located behind the facility. Surrounding the outdoor play area is a chain link fence that is at least 4 feet high. The outdoor play area is physically separate from the private elementary school ground with a chain link fence that is at least 4 feet high. LPA and LPM observed two gates that are locked which leads to the street. Applicant stated the gates will remain locked and will not be utilized as an entrance or exit, but only for emergency use. Applicant was advised to ensure the health and safety of children, and there must be visual supervision at all times during the use of the outdoor play area. The stairs to the outside area are fenced with a wrought iron fence with visibility strips on the steps to prevent tripping hazard LPA and LPM measured the area to include a total of 4,068.66 square feet and will accommodate the applicants request for 24 preschool children. There is one water fountain located outside and will be accessible to children during outdoor play times. There is sufficient soft poured-in-place rubber padding located around play structure. Per applicant, play structures are authorized for use of children ages 2-5, LPA and LPM advised applicant to follow the play structure’ manufacturers manual and for reference. There are two storage units located in the outdoor play area as well and contain play equipment for use by children. Platform on large play structure contains slight sloping wear and tear, LPA and LPM informed applicant to be aware of this.

LPA discussed the following: 100% supervision is required at all times, including in the bathroom; personal rights; inspection authority; reporting requirements; staff to children ratios and capacity; staff qualifications; and maintaining buildings and grounds. LPA discussed with Applicant any changes that may occur regarding the director or an employee acting in the director's absence must be reported to department within 10 working days.

Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Applicant was informed of Assembly Bill (AB) 2370, Chapter 676, Statues of 2018, requires all licensed child care centers (CCCs) constructed before January 1, 2010 to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).
