On July 16, 2024 at approximately 10:30am, Licensing Program Analysts (LPAs) Catrina Quimbo and Zeynep Basak conducted a pre-licensing visit to L'Academy Preschool program. LPAs met with applicant, Zhen Zhen Li, and facility director, Mysia Brooks, and explained the purpose of the visit.

Program is currently licensed for an infant program with a capacity of 18 children through facility 384004573. Applicant requested to add a toddler option component to licensed program. Applicant requested to add 12 toddler option children (18 months to 36 months) with 16 infants (6 months to 24 months).

Toddler option request was submitted to office June 19, 2024. As of this date, fire clearance has not yet been received. LPAs inspected facility for health and safety hazards and measured space to calculate capacity.

Applicant and facility director were reminded infant enrolled children and toddler option enrolled children must be kept physically separate and cannot co-mingle. Applicant was also reminded toddler option children must have written permission/consent from their authorized representatives to participate in toddler option component.

Indoors: There is one large classroom that is divided into two separate sections: infant area and toddler option area. The infant area was measured for 402.75 sq. ft. allowing for a capacity of 11 children. The toddler option was measured for 307.02 sq. ft. allowing for a capacity of 8 children. The total indoor square footage equals 709.77 sq. ft. allowing for a total capacity of 20 children.

The infant and toddler areas were observed to be well ventilated with appropriate temperature. Flooring in classroom includes carpets and padding for walking and crawling infants. LPAs observed flooring in classroom to be clean. Garbage bins in facility were observed to have tight fitting lids. Classroom includes appropriate toys and materials that were in good condition.

Facility includes a fully charged fire extinguisher, multiple first aid backpacks and a fire alarm system. There is sufficient storage for children's belongings located inside the facility, that is labeled with children's individual names and photos. Classroom is equipped with toys, materials and furniture that was observed to be in good working condition. LPAs did not observe any accessible poisons, cleaning solutions in the area. LPAs observed electrical outlets in facility to be made inaccessible with child safety covers.

Medications with children's individual names and required documentation will be stored in their cubbies. There is a refrigerator in the toddler option area that is available if medication requires refrigeration.

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. Younger infants sleep in cribs while older infants and toddler option children will sleep on cots. Crib area for infants is located in toddler option area. Applicant plans to install a wall partition to separate sleeping area from activity area. LPA reminded director children's sheets must be stored so that they are not touching each other.

There is one children's restroom in the facility that includes 2 toilets and 2 accessible sinks. Total allowable capacity for sinks is 30 children. Total allowable capacity for toilets is 30 children. There is a diaper changing table that is within arm's reach to an adult sink for diapering children.

Program currently provides a food service that includes a morning snack, lunch and afternoon snack. Water is made available to children through a pitcher that is refilled through facility's sink.

The requirement for Lead Water Testing was discussed (H&S Code 1597.16). LPA advised applicant to review PIN 21-21-CCP.

Applicant was reminded emergency disaster drills are to be conducted monthly and documented. Program signs children into and out of program by children's authorized representatives' electronic signatures. Printed sign in/out sheets are available if needed.

Outdoors: The facility does not contain any outdoor play area. The program currently has an open space waiver on file. Program is licensed to utilize the children's playground at Sergeant John Macaulay Park located on the corner of O'Farrell St. and Larkin St. which is 0.3 miles away from the facility. Applicant is to submit a current park permit to utilize park, to department. Children are walked to and from facility in a stroller with staff. Applicant and director were reminded ratio to park is 1:2 children in care.

LPA reviewed with director, the LIC311A, Records to Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Applicant was informed of the MyChildCarePlan.org site, a consumer education
website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with applicant, and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed applicant and director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Based on square footage, maximum number can be licensed for is 20 children with toddler option not to exceed 8 children. LPA will approve of toddler option component with infant license when following is complete:
-Updated LIC500 and LIC610.
-Installation of carbon monoxide detector.
-Fire clearance
-Park permit from San Francisco Parks and Recreation is submitted.
-Management approval

Exit interview conducted and report was reviewed with applicant, Zhen Zhen Li.
