25 | On August 18th, 2025 at approximately 1:00 PM, Licensing Program Analyst (LPA), Janet Gil, conducted a case management inspection and met with the director, Vivian Lopez. The purpose of the inspection was explained to the director. Present during today's visit, included 7 staff members (including director) and 20 enrolled children (7 toddlers and 13 preschoolers). Facility operates within its capacity and in accordance with the required ratio of staff to children. All adults present working in the facility have present criminal record clearance on file.
This case management inspection was related to the Unusual Incident Report (UIR) submitted by the director on August 8th, 2025. LPA discussed the nature of the incident during the inspection with the present director. The incident occurred on August 7th, 2025, when an enrolled child slipped and fell while playing in the outdoor area on platform planks. The fall resulted in a scrape under the child's nose and an injury to his mouth. Per director, two staff were present during the incident. The parent of the child was called, and the child was taken to ER after the incident. Per director, since the incident, the child has returned to school (on August 11th, 2025) and is well. Medical Doctor of Child recommended child to stay out of the sun, and possible glue needed for injuries.
LPA inspected the facility indoors and outdoors (including the area where the incident took place). Per director, facility plans to renovate the outdoor area through the Low-Income Investment Fund. Per director, renovation was set to start in July of 2025, but is running behind due to other projects. LPA reminded the director to make the department aware of when the renovation is complete for a follow up inspection.
No deficiencies were cited today under CCR, Title 22, Div. 12, Chapt. 1.

A Notice of Site Visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
