25 | On October 22nd, 2025, at approximately 9:20 AM, Licensing Program Analyst (LPA), Janet Gil, conducted a case management inspection and met with the lead teacher, Ricardo Ochoa. Director Vivian Lopez was not present during the inspection. Director communicated with LPA via phone call for additional information. The purpose of the inspection was explained to the lead teacher. Present during today's visit, included 7 staff members(including the lead teacher) and 25 enrolled children(8 toddlers and 17 preschoolers). Facility operates within its capacity and in accordance with the required ratio of staff to children. All adults present working in the facility have present criminal record clearance on file.
This case management inspection was related to the Unusual Incident Report (UIR) submitted by the director on October 13th, 2025. LPA discussed the nature of the incident during the inspection. The incident occurred on October 13th, 2025, when a mouse was found in the outdoor toy storage area for the toddler classroom. Per director's report, the mouse ran out of the facility and the children returned inside the classroom after the incident. Per lead teacher and director, since the incident, the facility personnel has been in contact with pest control and plans to place an additional box trap towards the exit area gate of the toddler outdoor play space. Director stated pest control will be coming into the facility on October 22nd, 2025, between 2 and 4 PM. Lead teacher showed LPA Gil additional mouse glue traps that have been placed around the facility in areas excluding children's classrooms/bathrooms. Per director, entrance/exit doors have had additional screens placed to not allow any rodents inside the facility.
LPA inspected the facility indoors and outdoors (including the area where the incident took place). Per director, facility has pest control come into facility monthly or as needed.
No deficiencies were cited today under CCR, Title 22, Div. 12, Chapt. 1.
A copy of today’s report was given to the lead teacher, Ricardo Ochoa.
A Notice of Site Visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
