On July 08, 2026, at approximately 12:00pm, Licensing Program Analysts (LPA) Ly, arrived at the facility unannounced to close the complaint investigation to the above allegations and met with the Director Sylvia Xureb. There were 6 staff members including the Director caring for 6 infants and 5 toddlers.

Based on information obtained during the course of this investigation through interviews, record review and information collected, there was no sufficient evidence to prove allegations (1) Staff are operating out of ratio, (2) Staff are operating over capacity, (3) Staff are comingling children. Although the allegation may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegations are deemed UNSUBSTANTIATED.

This report was reviewed with Director whose signature confirm have read the report. Report must be made available for public review upon request. A copy of this report and rights to comment and appeal have been discussed with the Director and left with the Director. Notice of Site Visit shall remain posted for 30 days.
