On August 11, 2026, Licensing Program Analysts (LPAs) Van and Tso conducted an unannounced annual inspection and met with Director Claudine Castillo. LPAs explained the purpose of the inspection, and the director granted access to the facility. The program operates as a single licensed combination center with infant and preschool components. The total licensed capacity is 32 children, consisting of 8 infants (ages 6 to 24 months) and 24 preschool-aged children (ages 2 years to entry into first grade).

At the time of the inspection, six teachers and the director were supervising 20 children, including 4 infants and 16 preschool-aged children. The center was operating within its licensed capacity and maintaining required staff-to-child ratios.

All staff present had criminal record clearances verified through the CDSS Guardian system.

The center operates Monday through Saturday 8:00 AM to 6:00 PM. The center uses the Brightwheel electronic sign-in/sign-out system, and attendance records confirmed proper sign-in/sign-out procedures.

LPAs and the director conducted a walkthrough of the facility to assess health and safety conditions. The center was observed clean, organized, and maintained at a comfortable temperature with adequate ventilation. Age-appropriate toys and educational materials were accessible, and furniture was sturdy and well-maintained. Foldable mats were available for preschool napping, and families provide bedding, which is washed weekly. Preschool restrooms were clean, stocked with sanitation supplies, and in good working order. A separate staff restroom is available on site. The center is equipped with dual centralized smoke and carbon monoxide detectors and multiple fully charged fire extinguishers.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

The preschool component has an approved waiver to use the public park at McCoppin Park playground for preschool outdoor activities. LPAs observed the park to include play structures and swings with resilient padding underneath. Equipment was securely anchored, trees provided shade, and the playground was enclosed by a fence at least four feet high. LPAs did not observe any bodies of water. LPAs reminded the director to follow all waiver conditions, including escorting preschool children to and from the park with adult staff, maintaining a ratio of one adult for every six preschool children, and ensuring visual supervision at all times during outdoor activities, transitions, and restroom use. Parents and volunteers may not be counted in the ratio.

Meals and snacks for preschool children are provided through an outside vendor or sometimes prepared on-site. Per the director, food preparation occurs on the second level of the facility in the owner’s licensed family child care home. Drinking water is readily available through water fountains and water pitchers with disposable cups.

A designated nap area is equipped with cribs, and foldable mats are provided only for infants capable of climbing out of a crib. Families provide sheets and supplies and wash them weekly. LPAs reminded the director that emergency exits must remain unobstructed and that cribs and mats may not block exit pathways.

The diaper-changing area contains a changing table with at least one-inch padding and three-inch raised edges. The changing table is located within arm’s reach of a sink and cleaned with disposable wipes after each use. LPAs reminded the director that the diaper-changing area and sink must be used only for diapering and not for food preparation or storage of classroom materials.

Infant food items were inspected and found properly labeled with each child’s name. Bottles were stored in the refrigerator at appropriate temperatures. According to the director, all bottles, nipples, and other infant feeding items are returned to families daily for cleaning. The director also stated that families provide food, formula, disposable diapers, and wipes for their infants. The center provides food for older infants and toddlers.

The infant program has an approved outdoor scheduled rotation waiver allowing no more than one infant on the patio/deck at a time. The wooden patio/deck provides a safe outdoor surface for infants and is free of gaps or protruding boards, with a smooth surface in good repair.

Sleep logs for the enrolled infants were reviewed during the inspection. According to the director, the center uses the Brightwheel application to maintain the 15-minute sleep logs. The logs were consistently documented and reflected the required 15-minute check intervals, each child’s name, the corresponding dates, and staff initials. LPAs recommended documenting when an infant is absent or on vacation. LPAs also reminded the director to maintain at least 30 days of logs on site and retain records for three years.

The center provides Incidental Medical Services. Children’s medications were stored securely, inaccessible to children, in original containers, unaltered, and accompanied by parental consent. Disinfectants, cleaning solutions, and other hazardous materials were stored out of children’s reach. The director stated that no firearms, weapons, pools, spas, or bodies of water are on the premises.

LPAs informed the director that the family child care home (384004833), located on the second level and operated by the same provider who owns the child care center, is a separate licensed facility. The center may not commingle children enrolled in the family child care home with children enrolled in the child care center. LPAs reminded the director that children enrolled in the center must remain in the center only, and children enrolled in the family child care home must remain in the family child care home only.

All required postings, including the Facility License, Outdoor Waiver, Notification of Parent Rights, Personal Rights, Car Seat Law, Emergency Disaster Plan, and Daily Activities, were posted.

The director stated that the center conducts fire and earthquake drills at least once every six months; the most recent drills were conducted on June 30, 2026.

During the inspection, LPAs reviewed a sample of children’s files from the infants and preschool components, as well as staff records. Children’s files were complete and included required documents such as immunization records, admission agreements, needs and services plan, emergency contact information, and personal rights forms. Staff files showed that all employees and individuals subject to background checks had obtained the appropriate criminal record and child abuse index clearances or exemptions. Multiple staff members held current CPR certifications, maintained required immunization records, and were up to date on mandated reporter training. The director’s and staff’s educational documentation confirmed completion of the required ECE units.

CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES: However, the test result has not yet been uploaded or displayed on the Waterboard website. LPAs informed the director and the center’s owner to contact the certified lead water tester to ensure the result is submitted and uploaded to the Waterboard website.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

The Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Childcare Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPAs discussed the safe sleep regulations with the director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPAs also informed the director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

The Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

LPAs discussed AB1207, the Child Abuse Mandated Training, with the Director. As of January 1, 2018, all staff must complete Mandated Reporter Training every two years. The training is available online at www.mandatedreporterca.com.

Pesticide regulations were discussed with the Director. In accordance with the Healthy Schools Act, California law requires that anyone using pesticides be trained annually in integrated pest management and the safe use of pesticides around children. The director was advised that a free one-hour online course is available on the Department of Pesticide Regulation's Web site: www.cdpr.ca.gov/schoolipm/training.
