An application for a large Family Child Care Home (FCCH) license, including a location change, was submitted to the San Bruno Regional Office. The facility was previously licensed under license number 384004330. The applicant plans to operate Monday through Friday, from 8:30 AM to 5:30 PM, caring for children from birth to 12 years old.

Background check clearances have been granted by the California Department of Social Services (CDSS) for the applicant and the applicant’s adult daughter. According to the application, the applicant’s adult daughter is the only other adult residing in the home.

The facility includes four bedrooms, two and a half bathrooms, a living room, a kitchen, a laundry room, and a boiler room. The designated daycare areas are the living room and the bathroom located within the living room. Off-limits areas include bedrooms #1, 2, 3, and 4; the kitchen; the laundry room; the boiler room; and two additional bathrooms next to the bedrooms.

The entrances to all four bedrooms, the two additional bathrooms, and the boiler room are connected by a hallway and secured with a safety gate at the entrance. The laundry room is inside the kitchen, which also has a safety gate at its entrance. A power box near the front entrance is secured with a safety strap to prevent children from accessing it. The two closets in the living room have child safety locks, and a second door leading to the street has a children's safety door lever to prevent children from reaching the street.

• Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.

• Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

The children’s bathroom was in working condition and equipped with appropriate toileting equipment. Cots and cribs were available for napping, and the applicant was reminded that all bedding equipment must be sanitized after each use.

The applicant confirmed that no weapons or firearms are present in the home. The applicant plans to use a designated cell phone that will remain in the home during daycare hours. LPA reminded the applicant that baby walkers, bouncers, jumpers, and similar items are prohibited.

The applicant plans to provide food service, including a morning snack, lunch, and an afternoon snack. Food storage, preparation, and sanitization practices were discussed during the inspection.

The applicant’s Pediatric CPR and First Aid certification is current until July 7, 2026, and Mandated Reporter training is current until October 15, 2026. LPA reminded the applicant that both trainings must be renewed every two years.

Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

LPA discussed with the applicant that the capacity specified on the license shall be the maximum number of children for whom care may be provided at any one time.

If the applicant provides care for a 7th and 8th child of school age, the applicant must use form LIC 9150 to notify the parents or guardians of all enrolled children. The Emergency Disaster Plan (LIC 610), Parent Rights (LIC 995A), and License (once received) must be posted in an accessible location where parents can see them.

LPA reminded the applicant that once she is licensed, she must be present at home and ensure that the children in her care are consistently supervised. Additionally, temporary absences should not exceed 20 percent of the daily care hours provided by the facility.

LPA reminded the applicant that once she is licensed, any duly authorized officer, employee, or agent of the Department shall, upon presentation of proper identification, enter and inspect any place providing personal care, supervision, and services at any time, with or without advance notice, to secure compliance with, or to prevent a violation of, the regulations adopted by the Department governing family child care homes

LPA informed the applicant that starting September 1, 2016, no one may be employed or volunteer at a family day care home without having been vaccinated against influenza, pertussis, and measles. LPA reminded the applicant that an individual who works in a family daycare home may submit a written declaration stating that they have declined the influenza vaccination. This exemption applies only to the influenza vaccine.

During today’s inspection, the applicant provided her lease agreement to the LPA.

The applicant provided proof of control of property.

Applies to large FCCH: Because the applicant rents/leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC 9151) that the applicant confirms was provided to the property owner/landlord. The applicant obtained a signed Property Owner/Landlord Consent form (LIC 9149).

Applies to small FCCH: The licensee has not obtained a signed Property Owner/Landlord Consent form (LIC 9149). Without this consent, the applicant understands that, once licensed, they can operate with a maximum capacity of 6 [or 12] children. If property owner/landlord consent is obtained in the future, the applicant is advised that a new Application for a Family Child Care Home License (LIC 279) must be submitted with a change of capacity fee of $25, to increase the capacity and provide care to 8 [or 14] children.

LPA discussed the safe sleep regulations with applicant, and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA also informed applicant of the importance of checking for and removing recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

On this date, 12/15/2025, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility address. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.

Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters, and other important information communication platforms.

LPA will not approve the large FCCH license until the following requirement(s) is/are met:

- Obtaining fire clearance from the local fire authority

The applicant requested a small FCCH license while waiting for a large FCCH license. The applicant provided a written request to the LPA. LPA will approve the applicant's request to operate as a small FCCH.

The applicant must abide by the small FCCH regulations.
