On June 2, 2026, at approximately 8:50AM, Licensing Program Analysts (LPAs) Tse, Tso, and Quimbo conducted an unannounced, prelicensing visit at the facility. LPAs met with Principal, Anna Tobin-Wallis, and explained the purpose of the visit. Present during the visit were 12 staff members and 24 preschool-age children. The facility’s hours of operation will be from 7:30AM to 5:30PM, Monday to Friday.

Program is an already licensed preschool program under license number 380504444. Program requested to add a toddler component. Due to the facility requesting an additional age component, a single license has been generated and the existing license number 380504444 will be consolidated to the single license number 384005182.

This facility submitted an application requesting 72 preschool-age children (2 years to 6 years) and 20 toddlers (18 months to 36 months). Fire clearance for a capacity of 96 children was granted on April 15, 2026.

The preschool classrooms will be Rooms 3, 4, and 5. The toddler classroom will operate in Room 6. The multipurpose room is currently licensed under the existing license number and will be undergoing renovations in Summer 2026. There is an existing transitional kindergarten program operating under the school district in Rooms 1 and 2.

Measurements were taken of each classroom, in addition to each outdoor play area.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Toddler Indoor Measurements
The toddler classroom was measured to contain a total of 798.48 sq. ft., which will accommodate the facility’s request for 20 toddlers.

Toddler Outdoor Measurements
The toddler outdoor play area was measured to contain 1504.52 sq. ft., which will accommodate the facility’s request for 20 toddlers.

Preschool Indoor Measurements
The preschool classrooms were measured to be 2459.67 sq. ft., which will not accommodate the facility’s request for 72 preschool-age children.

Preschool Outdoor Measurements
The preschool outdoor play area was measured to be 6689.57 sq. ft., which will accommodate the facility’s request for 72 preschool-age children.

Sinks and Toilets
There are a total of 8 toilets and 8 handwashing sinks in the facility, which will accommodate the facility's request for 92 children (72 preschool, 20 toddlers)

LPAs inspected the facility for any health or safety hazards. There are fully charged fire extinguishers that are at least size 2A10BC present. Emergency exits are clearly labeled and visible. Poisons, cleaning detergents, and other chemicals are stored inaccessible to children in care.

There are age-appropriate toys and learning materials present in the classrooms. There are cubbies available for each child to use for storage. Rooms 3 and 4 share a bathroom that contains 2 handwashing sinks and 4 toilets. Rooms 5 and 6 share a bathroom that contains 2 handwashing sinks and 4 toilets. Since Room 6 will become a toddler room, a waiver request for rotating use of the shared bathroom by the toddler and preschool room is required. There is a diaper changing station with an attached sink in Room 6.

LPAs observed that an electrical outlet panel in Room 5 was broken. LPAs advised that it be replaced. Carbon monoxide detectors in Rooms 3, 4, and 5 were observed to be non-operational. Carbon monoxide detectors shall be installed and operational.

Cots will be used for napping. Sheets will be provided by the facility. Blankets and other bedding will be brought from home and taken back once a week for cleaning. Additional blankets will be provided by the facility as needed. Laundry services are available on site as needed. Drinking water is provided by the facility and is provided from a non-contaminating fixture. Food is provided by a third-party vendor, and families have the option of providing food from home.

The outdoor play areas were observed to be free of debris and other loose articles. Outdoor play areas are enclosed by a fence that is at least four feet high. There are age-appropriate toys and equipment present. The toddler play area is kept separate from the preschool play area with a fence. LPAs advised that additional shade be provided and accessible while children are outside. There are no swimming pools or other similar bodies of water present in the facility.

LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP.).

LPA verified that the lead testing was completed in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP. Lead testing for facility was completed June 2022 and is within the 5.5ppb threshold.

CCC completed lead testing with no exceedances.

Facility representative was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.
