On 5/28/2026, Licensing Program Analyst(LPA) John Calandra arrived at the facility to deliver conclusionary findings for this complaint. LPA Calandra was greeted by Kelly Phillips, Health Services Director and explained the purpose of the visit.

Complaint alleged that financial abuse occurred to a resident living at the facility. According to the reporting party, a resident(R1) of the facility had money stolen by someone identifying themselves as an employee of the facility. According to the Administrator, the incident occurred on a day when the only management staff in the building was the Sales Director who sat in their own office. According to the Administrator they reviewed surveillance footage after the incident and did not see anyone other than the Sales Director enter the business offices area of the facility. Based on interviews and record review, no other individuals were seen in the facility’s business office on the day in question.

Although the above allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.

No deficiencies cited during today's visit. An exit interview was conducted and a copy of the report provided to facility representative.
