32 | According to interviews conducted, it was revealed that during this episode a patient from a mental health clinic approached a staff member in a verbally aggressive, and threatening manner. Additional staff arrived shortly and successfully intervened. Based on additional interviews conducted, staff member followed R1 from a safe distance, and no hands-on techniques for de-escalating behavior were used. Additionally, interviews conducted did not result in corroborated statements of R1 being threatened by staff members.
As a result, the preponderance of evidence standard is not met, and this allegation is UNSUBSTANTIATED. A finding of unsubstantiated means the allegation may have happened or is valid, but there is not a preponderance of the evidence to prove that the alleged violation occurred.
An exit interview was conducted with Program Director and a copy of this report was provided. Appeal rights provided. |
