Based on interviews with childcare director, childcare teachers, childcare children, and reporting party, LPA obtained corroborating information from childcare director, childcare teachers, childcare children, and reporting party. Multiple individuals stated that there were multiple occasions where childcare staff declined childcare children access to the bathrooms while they were in the teachers’ care, and childcare children had to wait to use the bathrooms when they needed to go to the bathrooms.
In addition, based on interviews with childcare director, childcare teachers, childcare child, and reporting party, LPA obtained corroborating information from childcare director, childcare teachers, childcare child, and reporting party. Multiple individuals stated there was an occasion where childcare staff declined C1’s access to C1’s sweater/jacket as C1 asked for the sweater and got cold while C1 played outside with other peers and teachers.
Based on corroborating information from the interviews of childcare director, childcare teachers, childcare children, and reporting party, there is a preponderance of evidence to prove the alleged violations did occur at the facility. Therefore, the above allegations are found to be SUBSTANTIATED.
The following Title 22 Deficiency is being cited on the subsequent 9099-D page. Upon receipt of Type-A citations, Facility representative/director shall provide copies of the LIC 9099-D for parents/guardians of children in care and for parents/guardians of newly enrolled children for the next 12 months. Facility representative must also keep the signed LIC 9224, acknowledging receipt of LIC 9099-D in each child's file.
Appeal Rights were provided, and LPA posted a Notice of Site Visit and 9099-D page dated January 28th, 2026, which must remain posted for 30 days.
An exit interview was conducted, and a copy of this report was provided and reviewed with the facility representative/director, Bonnie Cearley. A notice of site visit was provided and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
