25 | Licensing Program Analyst (LPA) Kesha Lewis conducted an unannounced Case Management visit to follow up on phone calls and text messages LPA Lewis received on 9/24/2023 from a resident. LPA explained purpose of visit to Administrator.
Resident was not at facility. LPA advised administrator to change lock that was damaged by resident because it is a fire hazard. LPA stated just to give resident notice to the resident regarding the lock. LPA will follow up with resident via phone call.
No Deficiencies were observed.
Exit interview conducted with administrator and a copy of report was given. |
