13 | Licensing Program Analyst (LPA) Kesha Lewis arrived at the facility unannounced to deliver complaint findings for the above allegation. LPA was greeted by staff and explained the reason for the visit.
LPA Lewis interviewed S2 during this visit. Based on interviews with staff and residents it is found that the allegation that the Facility is in disrepair is UNSUBSTANTIATED. Allthough the facility did have a heating unit that did break the facility offered numerus options for the five (5) residents affected and the heating unit was replaced within one (1) week and the adminstrator being notified.
Note that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, and therefore the allegations are unsubstantiated.
An exit interview was held and a copy of this report was given. |
