13 | On 08/11/2026, Licensing Program Analyst (LPA) Melina Oropeza arrived unannounced to conduct a complaint investigation. LPA met with Administrator Sheryl Bravo and explained the purpose of the visit.
During the investigation, LPA conducted interviews with the Administrator, 2 staff, resident (R1) and reviewed facility documentation, including staff training records. Information obtained from interviews and records reviewed did not determine that staff failed to provide adequate supervision to residents in care, failed to follow reporting requirements, or that staff were not adequately trained.
As a result, the preponderance of evidence standard is not met, and the allegations are UNSUBSTANTIATED.
A finding of unsubstantiated means that although the allegations may have happened or are valid, there is not a preponderance of evidence to prove that the alleged violations occurred.
An exit interview was conducted with the Administrator, Sheryl Bravo and a copy of this report was provided.
