13 | On 02/29/2024 at 8:00 AM, Licensing Program Analyst (LPA) Avelina Martinez and Licensing Program Manager (LPM) Liza King arrived at the facility unannounced to deliver complaint findings. LPA Martinez met with Daisy Aguilar and Judy Rodriquez and explained the purpose of today's visit.
Throughout the course of this investigation, LPA Martinez conducted interviews, reviewed facility records, and toured the facility. The investigation revealed there are nine residents on hospice. Based on six interviews, it was learned that residents and their responsible parties were able to choose their own hospice company. LPA Martinez was not able to make contact with three residents' responsible party, and LPA Martinez did not receive returned phone calls from them.
Due to the above noted information, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, and therefore the allegation is unsubstantiated. An exit interview was conducted, and a copy of this report was provided to the facility.
