13 | On 08/13/25, Licensing Program Analyst (LPA) Kesha Lewis made an unannounced visit to this facility to open an investigation into the above allegations. LPA identified herself upon arrival, stated the purpose of the visit, and asked to meet with the Designated Facility Administrator/Executive Director (ED). LPA met with Angela Riungu and a brief interview followed.
LPA requested a copy of log showing when the power went out and for how long it was documented that the power was out for 10 minutes and the generators came on a few minutes after.
The investigation revealed the preponderance of evidence standards have not been met; therefore, the above allegation is found to be UNSUBSTANTIATED. A finding that the complaint allegation is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. An exit interview was conducted, and a copy of this report was provided. |
