25 | On March 4, 2025, Licensing Program Analyst (LPA) Stacey Williams met with Facility Representative, Marti Rhoades for the purpose of a case management inspection. There were no children present during the inspection.
LPA discussed an incident report submitted to Community Care Licensing on February 6, 2025 involving a child that sustained an injury on the blacktop. Staff that was present during the incident was also interviewed during the visit. LPA also obtained medical documentation pertaining to the incident.
Exit interview conducted and report was reviewed with Facility Representative, Marti Rhoades. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
