On January 28th, 2025, at 10:50 AM, Licensing Program Analyst (LPAs), David Nguyen and Lauren Scott conducted an unannounced field visit to the facility. Both LPAs Nguyen and Scott were granted entrance into the childcare facility by facility representative/principal, Tamara Wells. LPAs Nguyen and Scott met with facility representative/principal, Tamara Wells to follow up on the Unusual Incident Reports (UIR) reported to Community Care Licensing Division (CCLD) on January 27th, 2025, via telephone.

LPAs Nguyen and Scott toured the facility, observed the care and supervision of children, reviewed records and conducted onsite interviews with teachers. During today’s field visit, LPAs Nguyen and Scott toured the facility and observed fifty-three (53) children being supervised by five (5) teachers.

Facility evaluation report was reviewed and discussed with facility representative/principal, Tamara Wells. Exit interview was conducted. Appeal rights and notice of site visit were provided. Facility representative/principal’s signature on this form acknowledges receipt of the form. A notice of site visit was posted and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

In the areas that were evaluated, no deficiencies were cited during today’s inspection.
