Licensing Program Analyst (LPA) David Nguyen met Principal Tammy Wells for the purpose of initiating and closing a complaint investigation. LPA was granted for entry into the facility by the principal. The purpose of the inspection was explained. LPA toured the facility and observed twenty-seven (27) children being supervised by seven (7) adults. LPA determined through accessing Guardian that all required adults were background cleared.

It was alleged, “Day care child sustained injury while in care.” and “Staff did not notify day care child’s parent of injury.” During the course of the investigation, LPA David Nguyen inspected the facility and interviewed the principal, the teachers, and the children’s parents. LPA also obtained information pertaining to the allegations.

Based on the information obtained throughout the course of this investigation, the above allegations could not be substantiated or dismissed. During the interviews, LPA Nguyen found conflicting information regarding the allegations that the day care child may have sustained injury while in care, and staff may have notified the child’s parent of injury. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur; therefore, the allegations are UNSUBSTANTIATED.

An exit interview was conducted in which the report was reviewed and discussed with Principal, Tammy Wells. LPA provided a copy of the report and Appeal Rights to the Principal. A Notice of Site visit was posted by LPA and Licensee understands it must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
