Licensing Program Analyst (LPA) David Nguyen and Licensing Program Manager (LPM) Chayntel Hunter met with Facility Representative/Admisnistrator Soni Singh for the purpose of closing a complaint investigation. LPA Nguyen notified facility representative of the complaint. There were sixteen (16) childcare children present and being supervised by three (3) childcare teachers. LPA Nguyen and LPM Hunter toured the facility inside and outside with facility representative and observed care, ratios, and children supervision during today’s inspection.

It was alleged that “Staff do not adequately supervise children.” During the investigation, LPA Nguyen requested facility roster and pertinent documents to assist with the investigation. LPA Nguyen inspected the facility and interviewed the childcare staff on site and reporting party via telephone.

Based on interviews with the director, administrator, teachers, assistant, and reporting party, LPA obtained corroborating information from childcare staff and reporting party. Multiple individuals stated that there was an occasion occurred on 11/20/2025, where a childcare child was not adequately supervised, resulting in them having access and drinking from a contaminated cup.

Based on corroborating information from childcare staff and reporting party and record reviews, there is a preponderance of evidence to prove the alleged violation did occur at the facility. Therefore, the above allegation is found to be SUBSTANTIATED.

The following Title 22 Deficiency is being cited on the subsequent 9099-D page.



A Type-A violation was assessed on a subsequent 809-D page. Facility representative was informed that this report dated 6/16/2026, documents one (1) Type-A citation related to children’s supervision which shall be posted for 30 consecutive days. The facility representative shall also provide a copy of this licensing report to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgment of Receipt of Licensing Report (LIC9224), or other written statement, must be placed in the child’s file for verification.

An exit interview was conducted, and the report was reviewed with Facility Representative/Administrator Soni Singh. Civil penalty was assessed for repeated violation for supervision. Appeal Rights were provided to facility representative. A Notice of Site visit was posted by LPA and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Staff Meeting will be held to focus on supervision. Administrator will email LPA the teachers’ meeting agenda and the sign-in sheet from this meeting. The teachers will also always provide adequate supervision while the childcare children are in their care.

Licensing Program Analyst (LPA) David Nguyen and Licensing Program Manager (LPM) Chayntel Hunter met Facility Representative/Administrator Soni Singh for the purpose of delivering the investigation findings for the above complaint allegations. LPA Nguyen disclosed the purpose of the inspection. LPA toured the facility inside and outside and observed care, ratios, and children supervision during today’s visit. There were sixteen (.16) childcare children present and being supervised by three (3) childcare teachers.

It was alleged that “Staff are not ensuring child is changed timely,” and “Staff did not notify parent of incident.” Throughout the investigation, LPA conducted the observations and interviews with the director, administrator, parents, and reporting party. During the interviews with childcare staff and parents, no information was disclosed to support the fact that staff were not ensuring child is changed in a timely manner and staff did not notify parents of incident.

The interviews produced conflicting information. During staff’s interview, LPA learned that childcare staff had and used diaper changing logs and changed childcare children who were not fully potty trained or who were partially potty-trained in a timely manner. In addition, LPA learned that childcare staff had and used the ProCare App to keep parents up to date with their children’s activities, incidents, and/or minor injuries. Parents who were interviewed expressed no concerns with children’s diapering needs satisfaction with the childcare service provided at the childcare center. Based on lack of collaborating information, the LPA is unable to determine whether staff were or were not ensuring child is changed timely and staff did or did not notify parent of incident.

Based on interviews and record review obtained from the reporting party, LPA David Nguyen determined that the complaint allegations were found to be UNSUBSTANTIATED, meaning although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur at the facility.
