Based on interviews with the director, administrator, teachers, assistant, and reporting party, LPA obtained corroborating information from childcare staff and reporting party. Multiple individuals stated that there was an occasion occurred on 11/20/2025, where a childcare child was not adequately supervised, resulting in them having access and drinking from a contaminated cup.
Based on corroborating information from childcare staff and reporting party and record reviews, there is a preponderance of evidence to prove the alleged violation did occur at the facility. Therefore, the above allegation is found to be SUBSTANTIATED.
The following Title 22 Deficiency is being cited on the subsequent 9099-D page.
A Type-A violation was assessed on a subsequent 809-D page. Facility representative was informed that this report dated 6/16/2026, documents one (1) Type-A citation related to children’s supervision which shall be posted for 30 consecutive days. The facility representative shall also provide a copy of this licensing report to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgment of Receipt of Licensing Report (LIC9224), or other written statement, must be placed in the child’s file for verification.
An exit interview was conducted, and the report was reviewed with Facility Representative/Administrator Soni Singh. Civil penalty was assessed for repeated violation for supervision. Appeal Rights were provided to facility representative. A Notice of Site visit was posted by LPA and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
