25 | On May 27th, 2026, at 10:00 am, Licensing Program Analyst (LPA) Janie Davis met with Director, Carmen Baugh. LPA toured the facility and observed thirty four (34) children being supervised by four (4) adult teachers.
Through interviews, LPA learned a child 4 (four) years of age was lelft unattended in a stairway for approximately three (3) minutes. This poses an immediate health, safety or personal rights risk to persons in care.
One Type A violation was assessed on a subsequent 809-D page. An exit interview was conducted, and the report was reviewed with director. Licensee Appeal Rights were provided to director. A Notice of Site visit was posted by LPA and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
LPA informed director, Carmen Baugh, that this report dated 05/27/2026 documents one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.
Also, LPA informed the facility representative to provide a copy of this licensing report dated, 05/27/2026, that documents any Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
