25 | On June 15th, 2023, at 1:30 pm, Licensing Program Analysts (LPAs), David Nguyen and Lauren Scott met with director, Merary Picos for an inspection of plans of corrections. Purpose of the inspection was explained. During today's inspection, LPAs inspected the day care areas. There were thirty-nine (39) children, being supervised by four (4) teachers at time of inspection.
On May 30th, 2023, the facility was cited two (3) Type A deficiencies due to inadequate supervision, child-teacher ratio, and co-mingling. LPAs observed that teachers provided supervision while childcare children were in their care. LPAs also observed that teachers maintained proper child-teacher ratio while the childcare children were in their care. LPAs also observed that the toddler classroom had no school-aged child or children in the toddler classroom while the childcare children were in their care. The deficiencies that were cited on May 30th, 2023 are cleared by today's field visit. A Proof of Correction letter was provided to S1.
On May 30th, 2023, the facility was cited for one (1) Type B deficiency due to childcare children were not sign in. LPA Nguyen received copy of the children’s sign-in and sign-out from site supervisor. The deficiency that was cited on May 30th, 2023 is cleared by today’s field visit. A Proof of Correction letter was provided to S1.
In the areas that were evaluated, no deficiencies were cited. An exit interview was conducted, and the report was reviewed, with Licensee. Licensee Appeal Rights were provided by LPA Nguyen. A Notice of Site visit was posted by LPA Nguyen and must remain posted for 30 days. Failure to comply with posting requirements will result in an immediate civil penalty of $100. |
