Licensing Program Analysts (LPAs) David Nguyen and Deborah Khashe met with licensee, Monica Storey, to deliver the findings of the complaint investigation regarding the above allegation. LPA Nguyen explained the purpose of the unannounced inspection. There were three (3) daycare children present at the time of the visit.

It was alleged that “Adult in the home yelled at the provider in front of day care children.” Throughout the investigation, LPA Nguyen conducted observations and interviewed licensee, licensee’s assistant, daycare parents, daycare children, and former daycare parents, and obtained pertinent information to assist with the complaint investigation. Based on interviews and record review, it was revealed that licensee’s assistant used hostile language and/or was verbally abusive towards the licensee in the presence of daycare children at the daycare facility. During today’s visit, LPA discussed with licensee some effective strategies for personal rights.

Based on the interviews conducted and record reviews, it was determined that the preponderance of evidence standard has been met; therefore, the above allegation is found to be SUBSTANTIATED. The following Title 22 Deficiency is being cited on the subsequent 9099-D page.

LPA David Nguyen informed licensee that this report dated 03/25/2026, documents one (1) type-A citation during the course of investigation. Type-A citation which shall be posted for 30 days consecutive days as there are immediate risks to the health, safety, or personal rights of children in care.

Also, LPA David Nguyen informed the licensee to provide a copy of this licensing report dated 03/25/2026 that documents any Type-A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement or Receipt of Licensing Report (LIC9224) or other written statement, must be in the child’s file for verification.

LIC9224 and Appeal Rights were provided. An exit interview was conducted, and the report was reviewed with licensee, Monica Storey. A Notice of Site visit was posted by LPA and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

LPA provided licensee with link for training video. Licensee agrees to watch Personal Rights Video and write a statement acknowledging she watched and understands the personal rights. Licensee also agrees to always provide a safe, healthful, and comfortable accommodations to the daycare children while they are in her care.

LPA Nguyen explained the purpose of the unannounced inspection. There were three (3) daycare children present at the time of the visit.

It was alleged that “Licensee did not report an unusual incident to licensing.” Throughout the investigation, LPA Nguyen conducted observations and interviewed licensee, licensee’s assistant, daycare parent, and daycare children, and obtained pertinent information to assist with the complaint investigation. Based on interviews and record review, it was revealed that licensee, Monica Storey, did witness an altercation between the licensee’s assistant and a daycare parent on October 17th, 2025, at the daycare facility, and licensee never notified the Unusual Incident Report (UIR) occurring on October 17th, 2025, to the California Department of Social Services (CDSS) and the Community Care Licensing Division (CCLD) within 24 hours and never submitted the written UIR to CDSS and CCLD within 7 days.

During today’s visit, LPA discussed with licensee, Monica Storey, the reporting requirements for the unusual incidents occurring in the Family Child Care Homes facility during the hours of operation that threatens the physical or emotional health or safety of any child to the California Department of Social Services (CDSS) and the Community Care Licensing Division (CCLD) within 24 hours and submit the written UIRs to CDSS and CCLD within 7 days to be in compliance.

Based on the interviews conducted and record reviews, it was determined that the preponderance of evidence standard has been met; therefore, the above allegation is found to be SUBSTANTIATED. The following Title 22 Deficiency is being cited on the subsequent 9099-D page.

Based on interviews with licensee, licensee’s assistant, and daycare parents, no information disclosed that the licensee’s assistant used hostile language or was verbally aggressive or physically aggressive towards a daycare parent in the presence of the daycare children at the daycare facility. LPA Nguyen was unable to determine if the adult in the home used hostile language or was verbally aggressive or physically aggressive towards the parent in the presence of daycare children. In addition, information obtained from the interviews revealed that children’s records are properly retained at the daycare facility. LPA Nguyen learned that there was not enough evidence to prove if licensee did not ensure children’s confidential information was kept secure at the daycare facility.

Based on conflicting information obtained from the interviews, LPA Nguyen is unable to determine whether the adult in the home threatened a parent in front of daycare children, and licensee did not ensure children’s confidential information was kept secure. Therefore, these complaint allegations were found to be UNSUBSTANTIATED, meaning although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur at the facility.



An exit interview was conducted in which appeal rights were provided and a copy of this report was provided and reviewed with the licensee, Monica Storey. A notice of site visit was provided and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
