On November 1st, 2023, at 9:30 am, Licensing Program Analyst (LPA), David Nguyen met with Licensee, Pamela Pettis Houston for the purpose of an unannounced annual inspection. The purpose of the unannounced annual inspection was explained. LPA was granted for entrance by the Licensee. Facility days and hours of operation are Monday through Friday from 7:00 AM to 6:00 PM when the school-aged children are in the school breaks--Spring Breaks, Winter Breaks, and Summer Breaks. Also the facility days and hours of operation are Monday through Friday from 3:00 PM to 6:00 PM when the school-aged children are attending school on the school days. The City of David Academy opens from January to December. There were no children present during today's inspection. Meals—breakfast and PM snack--were provided to school-aged children in care. AM snack and lunch were provided by parents and brought from home. Purified water from water service was provided for drinking water. LPA checked that annual fees are not current. LPA reminded the Licensee that she has an annual licensee balance. LPA gives the Licensee the fee balance and the PIN to pay the license annual fees.

LPA toured the facility inside and out. LPA observed that hazardous items (disinfectants, cleaning solutions etc.) were inaccessible to children in care. LPA reviewed first aid supplies, furniture, equipment, fire drills and drinking water. LPA observed all required forms to be posted. LPA observed functioning carbon monoxide and smoke alarms. There are adequate toys and equipment available for children. Outdoor play area was toured, the play structure appeared to be in good repair, and there is sufficient cushioning (wood chips) under the play structure. Due to no school-aged children present at the time of the inspection, LPA did not observe the sign in/out sheets. Licensee stated that the school-aged children were signed in by Licensee and/or childcare teachers and signed out by children's parents.

LPA reviewed the staff files. All staff present during today's inspection have a fingerprint clearance. The licensee was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA observed health screening reports with TB test and required MMR and TDAP vaccines for one (1) staff member. At least one staff member present today has current Pediatric CPR and First Aid. LPA observed AB1207 Mandated Reporter training certificates of two (2) staff members. The Child Care Provider Mandated Reporter Certification were verified and expired on 4/2023 and on 8/2023. The Licensee was reminded to renew the course every 2 years through www.mandatedreporterca.com website.

LPA discussed the Year-Round/Daily Reminder of Water Safety Requirements and Measures with licensee. LPA provided licensee the PIN 23-17-CCLD, the Year-Round and Daily Reminder of Water Safety Requirements and Measures.

This provider is not currently providing Incidental Medical Services (IMS) services to children in care. IMS policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.
