Two parents interviewed revealed a supervision concern. One of the parents stated her child was also bit. Another parent stated the “children run amok…chaos”.
On 8/13/24, LPA Beckby conducted a 2-hour observation of the facility. LPA’S observed the Director, Ms. Ella, sitting in a rocking chair. LPA observed a group of children in another area of the room, outside of Ms. Ella’s line of vision. LPA observed children wrestling, hitting each other, and pulling each other from an adult chair. Ms. Ella did not see these incidents, and therefore did not stop these behaviors.
Based on LPA observation of children being rough with each other as a result of the director’s lack of supervision, the preponderance of evidence standard has been met; therefore, the above allegation is found to be SUBSTANTIATED.
A Title 22 deficiency is cited on the subsequent page of this report. Licensee acknowledges, that for a TYPE A DEFICIENCY upon receipt, Licensee shall post LIC 9099-D with Type A deficiency for 30 days. Copies of this licensing report must be provided to parents/guardians of children in care at the facility, by the next business day and to parents/guardians of children newly enrolled at the facility during the next 12 months. LIC 9224 must be signed by parents/guardians and kept with the children's forms as a receipt whenever any Type A documents are provided by the Licensee.
On 5/23/24, Licensee was cited for the same deficiency (lack of supervision). Due to this repeat violation, a civil penalty is being assessed.
LIC 9224 and Appeal Rights were provided. The report was provided and reviewed with Director, Ella Temple Baumer. A Notice of Site Visit was posted by LPA’S which must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
