25 | On 05/01/24, Licensing Program Analyst (LPA) Elvira Sierra conducted a Case Management visit for the purpose of increasing capacity from 8 to 12 children and met with Licensee, Ja'nay Levingston. Fire safety inspection clearance was granted on 04/23/24 for a total capacity of 14 children. Hours of operation are seven days a week 24 hours a day.Licensee was advised never to exceed 24 hours of consecutive care. All individuals subject to criminal background review have obtained a criminal record clearance. Present in the facility was Licensee and Licensee's assisting caring for four children.
A health and safety inspection of all areas accessible to children was conducted and the following was observed. Licensee is requesting to make the Living Room part of the daycare area on today's inspection. New off limits areas are: All bedrooms, Garage, front yard, driveway, left, right and the grass area of the backyard. Licensee acknowledged that children may never enter these off-limit areas. Current CPR/First Aid certificate (expires: 06/2025). Roster of the enrolled children, and Fire Drill Log are all being maintained by the Licensee. Hazardous items were stored inaccessible to children in care. A functioning telephone, fire extinguisher (2A10BC), carbon monoxide and smoke detector were observed. No bodies of water were observed in the premises. The licensee was advised that prior to making alterations or additions to the home or grounds, the licensee shall notify the Department of the proposed changes. Large capacity options were reviewed with the Licensee. Licensee was advised to inform Licensing of any closing or absence that exceeds more than 20% of the time per day.
Effective today May 01, 2024, facility is approved for a Large Family Child Care License to serve 12 children (when there is an assistant present) with no more than 4 infants. Without assistance, the ratios revert to those for small family childcare homes. Property Owner/Landlord Consent form not on file.
In the areas that were evaluated, no deficiencies were observed at the time of the visit. During the exit interview, the LICENSEE, Ja'nay Levingston, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
Notice of site visit was posted and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Ja'nay Levingston. |
