On 02/21/2024, Licensing Program Analyst (LPA) Corina Beckby met with Licensee, Chiku Meher, for the purpose of an unannounced annual inspection. Licensee was present supervising 8 daycare children (ages 4 yrs old, 4 yrs old, 3 yrs old, 3 yrs old, 3 yrs old, 2 yrs old, 2 yrs old and 2 yrs old). LIC (126), Entrance Checklist for Family Child Care Homes, was provided and reviewed with Licensee. Facility hours of operation are Monday through Friday 6:00 am to 6: 00pm. Licensee has also applied to change from a Small Family Child Care Home to a Large Family Child Care Home. Fire clearance was granted by Tracy Fire Department – South County Fire Authority Community Risk Reduction on 02/20/24. LPA verified the annual fees are current.

Licensee stated there are no new residents in the home. All adults subject to criminal background review have obtained criminal record clearance. Licensee [or facility representative] was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

A health and safety inspection was conducted in all areas accessible to children. At the time of inspection, Licensee requested the backyard to be on limits. LPA checked for safety concerns and walked the perimeter of the fence. There are no sheds or bodies of water present. The backyard will be considered on-limits as of today. Licensee requested downstairs bedroom and full bathroom to be on-limits. LPA toured the room and the bathroom and approved them to be on-limits. Off-limits areas include: entire second floor, pantry, closet under staircase and garage. Off limit areas are being made inaccessible by closed doors, gates and supervision. Licensee acknowledged that children may never enter these off-limit areas.

Based on observation, the licensee did not comply with the section cited above in that Licensee was caring for 8 children (2 were 4 years old, 3 were 3 years old and 3 were 2 years old) under the age of 5 by herself, which poses an immediate health, safety or personal rights risk to persons in care.

There will be a minumun gap of 15 minutes betwen transition time between morning session that begins at 8:30 am to 12:50 pm and afternoon session at 1:beginning at 1:05 pm ending at 6:00 pm. Licensee will impose late fees. Licensee will remind families to drop off and pick up on time and will not permit families to drop off early or pick up late between sessions. Licensee will provide LPA Beckby with an AM & PM schedule for all children.

LPA observed the required postings and a working phone and 3A40BC fire extinguisher. LPA observed functioning dual smoke and carbon monoxide detector. LPA toured the kitchen area and verified knives were inaccessible to children in care. Licensee stated there are no weapons.

LPA reminded Licensee of ratio regulations for a small family child care home. Licensee was out of ratio at the beginning of the inspection; however, 2 children left during the inspection. LPA observed play room areas with age-appropriate toys for children. Licensee is responsible for ensuring all child proof locks and gates are secured and in working order. Licensee understands that prior to making alterations or additions to the home or grounds, the licensee shall notify the Department of the proposed changes.

5 Children's files were reviewed. Emergency information and required immunization records were on file. LPA observed a current roster and documentation that a fire drill is conducted at least once every six months. Licensee's immunization records are available in the facility file. Current EMSA pediatric CPR and First Aid certification was verified and expires on 12/03/24. Licensee has current Mandated Reporter Training Certificate that expires 01/05/25 and understands Mandated Reporter Training must be completed every two years starting January 1, 2018 and retain proof of completion in the facility file. Licensee has no infants in care. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA discussed the requirement to check and log infant napping every 15 minutes for infants under 24 months.

LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

(R&Rs) throughout California. During the exit interview, Licensee, Chiku Meher, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

LPA informed Licensee, Chiku Meher that this report dated 02/21/24, documents a Type A citation that is an immediate Health and Safety, or Personal Rights risk to persons in care. An 809D is issued for the deficiency.

Upon receipt of a Type A deficiency licensee shall post the report for 30 days in addition to the Notice of Site Visit and provide copies of the licensing report to parents/guardians of children in care at the facility. This report must be provided to parents/guardians of children newly enrolled at the facility during the next 12 months and licensee will obtain a signed Acknowledgment of Licensing Reports (LIC9224) from parent/guardian and place it in each child's file. If these requirements are not met, civil penalties will be assessed.



An Exit interview was conducted, and the report was reviewed with Licensee, Chiku Meher. LPA posted a notice of site visit. Licensee understands the Notice must remain posted for 30 days and that a failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeal Rights were provided. A copy of this report will remain on file for a period of three years for public review upon request. The licensee's signature on this form acknowledges receipt of this form.


Effective today 02/21/2024, facility is approved for a Large Family Child Care License to serve 12 children (when there is an assistant present) with no more than 4 infants, or capacity of 14 children when 1 child is enrolled in Transitional Kindergarten or above and 1 child at least age 6 with a maximum of 3 infants. Without assistant, the ratios revert to a small family childcare home.
