25 | On November 26, 2024, Licensing Program Analyst (LPA) Corina Beckby met with Applicant (A1) Porscha Gulley for the purpose of a pre-licensing and change of location inspection. LPA provided the Entrance Checklist to A1 and reviewed the LIC 311D, Forms/Records to Keep In Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted. Fire Inspection Clearance was granted on 10/30/24 by The Tracy Fire Department County Fire Authority Community Risk Reduction for a total capacity of 14 children.
Applicant and all adults living in the home have criminal background clearance and was verified by LPA accessing Guardian. A1’s tentative hours of care are 7 days a week 23 hours a day.
Because A1 rents the home, proof of landlord notification is required. LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms the form was provided to the property owner/landlord. A1 has not obtained a signed Property Owner/Landlord Consent form (LIC9149). Without this consent, the applicant understands that, once licensed, they can operate with a maximum capacity of 12 children. If property owner/landlord consent is obtained in the future, the applicant is advised that a new Application for a Family Child Care Home License (LIC 279) must be submitted with a change of capacity fee of $25, to increase the capacity and provide care to 14 children.
A health and safety inspection were conducted inside and outside of the FCCH. A1 accompanied LPA for the entirety of the tour. The FCCH is a two story home with 5 bedrooms, 3 bathrooms, family room, dining room 1 &2, sitting room, kitchen, laundry room, hall closet and garage. The OFF-limit areas are second floor, garage, downstairs bedroom, all closets, right side of back yard, shed and front yard.
